STATEMENT OF WORK TEMPLATE A.GENERAL INFORMATION 1.Service Contract for Zeiss Microscope Imaging System 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The overall purpose of this contract is to provide full coverage for parts and labor as detailed below for the Zeiss Microscope Imaging System located in Dr. V Haroutunian's research program in room 4F-28A of the Research Building at the JJP VA Medical Center, Bronx, NY 10468. 3.Background: This service contract a laser microscope imaging system consisting of Laser Microscope 6FTSS355-50 OEM and Laser Interface MB IV C MM042017. The cost breakdown is as follows: Laser Interface MB IV C Serial #MB042017 Laser 6FTSS355-50 OEM: Microscope coverage includes: 1.Basic Microscope Stand 2.Microscope Electronics and Firmware 3.Illumination System, 4.Objectives (1) 5.Filters (1) 6.Manual Stage Laser Interface MB coverage includes: 1.Motorized Scanning Stage and Motor control unit 2.Apotome 3.Incubator 4.AxioXam CCD Camera 5.Zeiss System Computers (3) 6.Vivatome 7.Tirf (2) 8.Spinning Disk Service contract coverage is included in the Statement of Work below. 1= 4.Performance Period: 2/1/2016 through 1/31/2021 + 5 option years 5.Type of Contract: Firm-Fixed-Price B. STATEMENT OF WORK 1.Scheduled and emergency service performed between the hours of 8 am and 5 pm Monday through Friday, excluding Carl Zeiss Micro imaging, LLC holidays 2.All travel charges within the continental USA ·3. One (1) preventive maintenance inspection per Service Agreement year 4.All covered replacement parts 5.Applications Support/Training 6.Software updates when available 7.Factory trained Field Service Representatives Contracting Office Address: VISN 3 Network Acquisition & Logistics James J. It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to this announcement must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at https://www.vip.vetbiz.gov/.
