- W-9 FORM MTC VENDOR SBE CERTIFICATION FORM ACCEPTANCE OF MTC T&CS FORM MTC TERMS & CONDITIONS SERVICE CONTRACT ACT WAGE INFORMATION THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Clearfield Job Corps Center. MTC is requesting bids for Janitorial Services (Wellness Center only) at 20 West 1700 South, Clearfield, UT 84016. The Request for Quote (RFQ) contains all requirements and the statement of work (SOW). 1. Please contact Christy Howard, 801-416-4260 if you would like a “non-mandatory” site visit prior to the bid due date of Tuesday, 2/2/16 at 2:00pm (MST). 2. All bids and associated “attached” documents are due no later than Tuesday, 2/2/16 at 2:00pm (MST) via email to sandra.clore@mtctrains.com. 3. Services provided under this solicitation are under N AICS Code 561720 Janitorial Services. Information regarding this NAICS code is located at http://www.census.gov/cgi-bin/sssd/naics/naicsrch MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final.
