Description
THIS IS A SOURCES SOUGHT NOTICE. This is not a request for quote. A solicitation will not be issued at this time. The Department of Veterans Affairs, West Palm Beach (WPB) VAMC has a requirement for Home Oxygen and Durable Medical Equipment Services (DME). The intent of the procurement is to award an Indefinite Delivery Indefinite Quantity contract (IDIQ). The performance period will consist of one base period. Scope of Requirement: There were 670 prescriptions referrals for Home Oxygen Services/DME Set-up and made by WPB VA in the past fiscal year. The Performance Work Statement (PWS) defines the effort required for the setup, installation, and management of Home Oxygen and Durable Medical Equipment and/or Ventilation services, in accordance with Joint Commission (JC) standards, to any Patient that are treated at the main WPB Medical Center (VAMC) and all Community Based Outpatient Clinics (CBOCs) and Outpatient Clinics (OCs) supported therein. WPB VA does not have the capacity to provide these services so a contract will be sought for the provision of home oxygen, durable medical equipment and all associated products and services. All services shall be performed in the veteran's place of residence or in a nursing home housing the veteran. The area of service shall be all patients serviced by VA WPB, including catchment areas, regardless of where Patient resides. The equipment needs are subject to change as determined by the prescribing physician. Contractors shall have a facility or resources physically located in the geographic area in which they shall provide service. Non-territory requests will be required from time to time and the Contractor shall accept these "non-territory" veterans at that Contractor's awarded price. VA reserves the right to assign these Patients to any contractor. The geographical boundaries described herein have been determined as the areas of responsibility/jurisdiction for each VA West Palm Beach: Glades, Hendry, Indian River, Martin, Monroe, Okeechobee, Palm Beach, and St. Lucie counties. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance of residential renovation projects (including references); (3) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (4) documentation on company's bonding capacity and limitations; and (5) any other pertinent company documentation. The response date to this Sources Sought notice is February 29, 2016 at 4:00 pm ET time. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or sole-source procurement is appropriate. The Government will not pay any costs for responses submitted. Electronic submissions are requested via Mayra.rosa@va.gov. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION.
