Description: Synopsis/Solicitation for Karl Storz Scopes or Equal. (I)This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA247-16-R-0206 (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. (IV)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (V)The Network Contracting Office 7-SAO EAST intends to award a firm-fixed price commodities contract for Karl Storzs Scopes. (VI) Located at the Atlanta VA Medical Center, 1670 Clairmont Road, Decatur, GA 30033. (VII)The North American Industrial Classification System (NAICS) code for this procurement is 339113 with a small business size standard of 500 employees. This procurement is restricted to and for all eligible veteran owned small business. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. (VIII)Questions concerning this solicitation shall be addressed to Demetrius D. Kittrell, Contracting Specialist at Demetrius.Kittrell@va.gov. All questions or inquires must be submitted no later than 11:00 AM Eastern Standard time on February 05, 2016. No phone calls will be accepted. (IX)The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price and technically acceptable which are expressed in the requirements of the SOW. Quotes are due by noon Eastern Standard time on an February 21, 2016. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Demetrius.Kittrell@va.gov. (X)This combined solicitation/synopsis is for the purchase of the following commercial items: STATEMENT OF WORK 1.0 BACKGROUND The facility Operating Room requires Karl Storz rigid endoscopic scopes and other related equipment for minimally invasive procedures to replace the 15-year-old scopes the Surgical & PerioperatiAve Service Line had. Gynecologic surgical cases often include laparoscopic and hysteroscopic procedures provide findings, which are imperative to relay to the patient and often their family. 2.0SCOPE The vendor/contractor shall provide a Karl Storz rigid endoscopic scopes and other related equipment for this purchase. 3.0VENDOR/CONTRACTOR REQUIREMENTS The vendor/contractor shall ensure the Karl Storz rigid endoscopic scopes and other related equipment to meet specifications. TASKS The contractor shall complete the following tasks in support of this Delivery: 01.No installation provided by vendor; the delivery will be by third party entities FedEx, USPS, UPS etc. 4.0DELIVERABLES The vendor/contractor agrees to the following deliverable dates: 01.The vendor shall deliver the equipment within seven 7-10 days after the award. 5.0PERIOD OF PERFORMANCE $ COSTING See Table Below. 6.0PLACE OF PERFORMANCE Atlanta VAMC, Building C, Operating Room, 2nd floor; 2c104 7.0CLASSIFICATION/SECURITY Security clearance is not required.
