- Accident Prevention Plan Wage Determination Request for Proposal This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The attached solicitation document and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. Please find the attached formal request for proposals (RFP). This procurement is 100% small business set-aside. The solicitation number for this RFP is W913E5-16-R-0001. The associated North American Industry Classification System (NAICS) code is 811310 - Commercial and Industrial Machinery and Equipment Repair and Maintenance, which has a size standard of $7,500,000. The U.S. Army Engineer Research and Development Center (ERDC), Cold Regions Research and Engineering Laboratory (CRREL) has a requirement for yearly maintenance, repair, and inspection of four (4) passenger elevators and one (1) freight elevator at its Hanover, NH facility. This RFP will result in the award of a REQUIREMENTS CONTRACT for one (1) base year and four (4) option years. The contractor shall provide services and materials necessary to accomplish the tasks described in Section C, Performance Work Statement (PWS). Evaluation Factors: Technical capability to meet the government's requirements; price; past performance. See Section M of the attached RFP for evaluation criteria. Technical Capability: a.Proposals must demonstrate that an offeror has at least 10 years of experience in elevator maintenance, repair, and inspection. b.The proposal shall demonstrate and certify that the offeror can adhere to the response times for both routine and emergency service calls indicated in PWS Section 9. c.Offeror must employ a New Hampshire Licensed Elevator Mechanic and must provide evidence as such with proposal. d.Offeror must employ a New Hampshire Licensed Elevator Inspector and must provide evidence as such with proposal. Past Performance: Proposals must include at least one (1) customer reference for the same or similar work performed within the past one (1) year. Pricing: The proposal shall include prices for each contract line item number (CLIN) located in Section B, except those for Unplanned Repairs Labor and Material (see next paragraph). Failure to submit prices for any of the items required will deem a proposal non-responsive and no consideration for award will be given. The fixed prices proposed, if deemed fair and reasonable, will become part of the award.
