Description
Atch 6, EMS Flyer Atch 5, Historical Data Atch 4, Performance Management Plan Atch 3, Applicable Clauses and Provisions Atch 2, Performance Work Statement Atch 1, Pricing Schedule (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA5613-16-R-0013, and the solicitation is issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85. (iv) This is not a set-aside. The North American Industry Classification System (NAICS) code for this solicitation is 811310. (v) The 700 CONS/LGCA Flight has a requirement to purchase Preventative Maintenance, Safety Inspections and Repair of Hoists in the Kaiserslautern Military Community (KMC), for the period 01 April 2016 through 31 March 2017, with options to extend the performance through 31 March 2020. Offerors shall use Attachment 1, Pricing Schedule, which includes all contract line item numbers, quantities and units of issue, to fill in all unit prices and total amount blocks, for the base period and all option periods. (vi) Attachment 2, Performance Work Statement (PWS), contains the description of the requirement for Maintenance, Safety Inspections and Repair of Hoists in the Kaiserslautern Military Community (KMC). Attachment 5, Historical Workload contains the historical workload data for repair calls. (vii) Places of performance and acceptance are identified in Appendix B of the PWS, contained in Attachment 2. All offerors must propose FOB (free on board) destination. Required period of performance: Base Period: 01 April 2016 - 31 March 2017 1st Option Period: 01 April 2017 - 31 March 2018 2nd Option Period: 01 April 2018 - 31 March 2019 3rd Option Period: 01 April 2019- 31 March 2020 (viii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (Oct 2015), applies to this acquisition. Addenda to this provision are included in Attachment 3, Applicable Clauses and Provisions. (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items (Oct 2014), applies to this acquisition. Specific evaluation criteria contained in paragraph (a) of that provision are included in Attachment 3, Applicable Clauses and Provisions. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Nov 2015), Alternate I (Oct 2014), with their offer, or complete paragraph (b) of 52.212-3, if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov. The provision is included in Attachment 3, Applicable Clauses and Provisions. (xiii) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. (xiv) In order to be eligible for award the contractor is required to be registered in the System for Award Management (SAM), http://www.sam.gov, and Wide Area Workflow (WAWF), https://wawf.eb.mil. (xv) Offers are due 17 February 2016 not later than 15:00hrs (3:00 pm) Central European Time (CET). Offers may be sent via email to erica.morris.4@us.af.mil and ingeborg.jenkins.1.de@us.af.mil, or hand-delivered to the following address; 700th Contracting Squadron/LGCA, Kapaun Air Station, Bldg 2767, 67661 Kaiserslautern. Emailed proposals must be received in at the stated addressess' email inboxes on time and the addressee must be able to open the email and all attachements. No other email receipt will be acceptable. It is the offerors' responsibility to confirm the timely receipt of the proposals by the stated addressees. Note: Kapaun Air Station is an Air Force controlled installation. Delays at the gate will not be considered if offers are late. Further details can be found in the addendum to 52.212-1 - Instruction to Offerors, in Attachment 3 (xvi) A site visit is not contemplated at this time, however contractors may contact the point of contact for this solicitation to schedule a site visit, if needed.
