- Self Certification Standard Terms Tax Form THIS IS A SUBCONTRACTING OPPORTUNITY Long Beach Job Corps opaerated by Odle Managment under DOL contract DOL-ETA-16-H-004 is seeking a vendor to supply paper and other products to be used in the Center Cafeteria. A list of those products are: 6" Foam Plate (Desert Plate) 12 oz Foam Bowl 16oz Foam Cup, 1000 per case Plastic Fork, 1000 case count Plastic Spoon, 1000 case count Napkins Torks Express 12x500 per case made by recycled fiber Aluminuim Foil, 18" x 100 Ft Plastic Wrap, 18"" x 2000 Ft Liner, Paper Full Sheet Pan, 1000 per case 10lb Lunch Bags, 500 per case, 6 5/16" x 4 3/16" x 13 3/38" Plastic Aprons Disposalbe, 28" x 46", 100 case count To Go Boxes, 9x0 Foam Box Clamshell All bids must refer to the list provided and must include the price per item/case. A sample of each item may be request from the winning vendor to show quality. A quote must include any shipping or frieght charges. All deliveries must arrive at the center between the hours of 8:00 AM to 5:00 PM, Monday through Friday. No deliveries well be accepted during the lunch hours of 11:00 AM to 1:30 PM. Vendors are welcomed to provide catalogs of other items that may be purchased for use in the cafeteria. Final decisions do not solely rest on the price per item but will include quality of products and the adherance of delivery expectations. A BPA (Blanket Purchase Agreement) will be issued for the purchase of all items. Prices must be valid through the end of October 2016.
