- This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with information in FAR Subpart 12.6, using Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ). PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The RFQ number is VA244-16-Q-2084. The Offeror must quote on an all or none basis. The government will award a purchase order on the basis of the lowest priced, technically acceptable offer. Since an award will be based on initial responses, offerors are highly encouraged to quote their most competitive pricing in their initial response. An award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106-2. This announcement will close at 10:00am on 02 February 2016. To be eligible to receive an award resulting from this RFQ, Offerors must be registered in the System for Award management (SAM) database, no exceptions. To register, please visit http://www.sam.gov. All quotes shall include a CAGE Code, DUNS, and Tax Identification Number. The NAICS code is 339112 and the Small Business Standard is 500 employees. This is a 100% small business set-aside action and the award will be made on a Firm-Fixed Price purchase order. Statement of Work Rolling Stock Quarterly Cleaning, Wilmington VAMC 1.Introduction. The purpose of this statement of work is to provide qualified and approved contractors the scope of work for cleaning equipment at the Wilmington VA Medical Center. 2.Work to be Performed 2.1.Quarterly deep cleaning of stretchers, wheelchairs, and over bed tables using Safe Wash detergent with the Quaternary disinfectant and CB2 adhesive remover. 2.2.Above items will be picked up and returned to pre determined units 2.3.Work will be performed on site and all waste water will be properly treated offsite at the responsibility of the contractor. 2.4.Items will be tagged as cleaned when work is complete 3.Location of the Work Wilmington VA Medical Center 1601 Kirkwood Highway Wilmington, DE 19805 Work to be done on the back dock of building 1 on a box truck provided by the company. Management will be notified when equipment will be cleaned. 4.Period of Performance and Timeline 4.1.It is anticipated that the project will take 2 days (@ 8 hours/day) to complete and will be performed in the winter, spring, summer and fall of each year. 4.2. Equipment will be cleaned on Saturday and Sunday 4.3Work should begin no later than 30 days after the notice to proceed after receipt of contract. Anticipated start date as 02/01/2016 - 01/31/2017 with three, one year option periods. 5.Special Requirements 5.1.Coordinate all contractor personnel with Wilmington VA Medical Center to ensure all safety procedures are met. 5.2.Equipment used must be intrinsically safe. 5.3.Contractor will be liable for any discharge into onsite storm water or waste water conveyance and must immediately notify Safety & occupational health of any occurrence. 5.4.Contractor must receive badge from the police before work is started 6.Scope of Work. A.Equipment service includes functional inspection of all components, disinfection and cleaning, and decontamination. Services shall be performed quarterly on each piece of equipment designated by contracting authorities. Current workload is estimated at forty (40) stretchers, sixty-four (64) wheelchairs, and one hundred and thirty-five (135) over the bed tables and may vary quarterly based on resident population. B.Each designated wheelchair will be cleaned, rinsed, dried, decontaminated, disinfected, and lubricated concurrently with the manufacturer's safety and functionality check. This applies to all electrical and non-electrical. C.Once inspected, decontaminated, and disinfected, designated equipment will be tagged with an identifier that can be tracked for quarterly cleaning. D.The contractor will establish the identification system, using bands or labels to identify equipment for the purposes of service rotation. E.The contractor shall provide reports upon completion of service indicating the level of functionality for each piece of equipment. All equipment removed from service for additional repairs or replacement parts will be reported on a daily basis. Contractors shall not conduct repairs beyond the scope of their expertise and will refer to the model's manufacturer for follow up maintenance or repair. The report will indicate all servicing and parts required beyond the equipment's normal cleaning and decontamination cycle. F.Cleaning and decontamination includes but is not limited to the following general tasks: 1.Clean, rinse, dry, and lubricate each wheelchair or apparatus. 2.Remove hair, string, and sutures from wheel axles. 3.Perform minor adjustments such as tightening screws, bolts, and rails. 4.Provide a list of replacement parts or extensive repair work orders for additional resources, funding, and submission to the Government for the equipment's manufacturer or vendor. G.The contractor shall be responsible to clean up their designated work area and return equipment to the respective owners prior to departing the premise. Under no circumstances will the contractor depart without equipment being returned to the respective owner. Equipment designated as "Unsafe" or "Awaiting Repair" shall be tagged as such and returned to Bio Med the until the manufacturer's service representative or repair parts have been received. H.The contractor shall invoice quarterly. Invoices shall include (at a minimum) the actual service date for each piece of equipment, make, model, and serial number of the equipment, repair parts installed, equipment status, and current purchase order number. Equipment marked "unserviceable" will require a listing of all parts that the WVAMC must order prior to the equipment being returned to a serviceable condition. 7.Equipment and Supplies. The contractor's proposed list of cleaning products and decontaminants shall first be presented to the WVAMC Infection Control and Safety Officials for approval of use at this facility. The contractor shall be responsible for obtaining Material Safety Data Sheets (MSDS) for each item and submitting them to Infection Control and Safety & Occupational health prior to utilizing any decontaminant or disinfectant or other material. When possible, the contractor must utilize EPA Environmental Preferable Purchasing (EPP) products found at http://www.epa.gov/epp/. 8.Access to the Wilmington VA Medical Center. A. All contractor personnel accessing federal facilities must be fingerprinted and provided an access badge that shall be displayed above the waist at all times while in the facility. The EMS leadership shall sponsor contractors and prepare VA Forms 0711, and contractors shall submit fingerprints that will be verified prior to commencing any activity within the facility. B.Upon termination of the contract or any contractor employees, all access badges will be returned to the VA Police, and further access to the facility will be denied. C.The VA Police have the right to refuse access to any contract employee at any time without further notice. 9.Quality Control. SPD, Nursing, and EMS staff will spot check the suitability of work performed under this contract during normal clinical hours. Deficiencies will be reported to the contractor. The VA expects at least 95% of all equipment to be maintained, cleaned, and disinfected at all times. Continued unacceptable performance under this contract may result in termination.
