- COMBINED SYNOPSIS/SOLICITATION Provide all labor, tools, materials, equipment, facilities, transportation and supervision necessary to maintain and repair government owned equipment listed in Attachments 1, Exhibit A located at the Alexandria, LA VA Medical Center (VAMC) and Attachment 2, Exhibit A located at the New Orleans, LA VAMC, in complete and strict accordance with the resultant contract/order and all attachments thereto. (I) This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. All businesses submitting an offer/quote must be registered on https://www.sam.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes referencing RFQ Number VA256-16-Q-0265. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85A. (IV) This procurement is unrestricted. The associated NAICS code is 811219. The small business size standard is $20,500,000.00. (V) This requirement is for preventive maintenance and other services as outlined in the Statements of Work (Attachments 1 and 2) for anesthesia equipment. 1. Schedule of Services (Pricing): SEE ATTACHMENTS 1 AND 2, EXHIBITS A (One for each attachment) (VI) Description of requirements is as follows: Salient Characteristics: SEE ATTACHMENTS 1 and 2 (VII) The required performance period is for a base period of 3/1/2016 (Est.) thru 9/30/2016, with two (2) one year option periods for a continued performance period of up to 10/1/2016 thru 9/30/2018. Place(s) of performance is/are Pineville and/or New Orleans, LA. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items, applies to this acquisition. (IX) Evaluation - This acquisition will be conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 13.106-2, and as supplemented by agency procedures. (X) The offeror must submit a completed copy of the provision 52.212-3, Offeror Representations and Certifications - Commercial Items, with its quote (SEE ATTACHMENT 3, SECTION D.2). An offeror shall complete only paragraph (b) of provision 52.212-3 if the offeror has completed the annual representations and certifications electronically at https://www.sam.gov. If an offeror has not completed the annual representations and certifications electronically at the above website, the offeror shall complete only paragraphs (c) through (o) of this provision. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition (see Attachment 3, Section C for addendum to this clause). (XII) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition (SEE ATTACHMENT 3, SECTION C.6 FOR THE SPECIFIC CLAUSES THAT APPLY). (XIII) Additional Terms and Conditions applicable to this acquisition: SEE ATTACHMENTS 1, 2, and 3. (XIV) All interested parties must submit all questions concerning this solicitation in writing via email to the Contracting Officer. Questions must be submitted by email to james.hunt2@va.gov, and received no later than 12:00 PM, February 10, 2016. (XV) Quotes are required to be received in the contracting office no later than 10:00 A.M on February 17, 2016. In order to be considered for award, quotes must include the following: (1) Completed Attachment 1, Exhibit A; Attachment 2, Exhibit A; and, Section E.2 of Attachment 3 (see item (X) above). (2) A technical capability statement addressing each of the following: (i) Required certification of service technicians (Sections 2.5 of Attachment 1 and Section 4 of Attachment 2); (ii) Familiarity/expertise with OEM recommendations for preventive maintenance of covered equipment (Section 3.2 of Attachment 1; and Sections 3.2 and 3.3 of Attachment 2); (XVI) All quote materials must be emailed to the attention of Jim Hunt, at james.hunt2@va.gov.
