- RFQ-RT-16-00026 Statement of Work Clauses THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FEDERAL ACQUISITION REGULATION (FAR) SUBPART 12.6 "SOLICITATION FOR COMMERCIAL ITEMS", AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation number is RFQ-RT-16-00026 and the solicitation is being issued as a 100% small business set aside Request for Quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-77. The Environmental Protection Agency (EPA) anticipates that a purchase order will be awarded as a firm-fixed price purchase order using FAR Part 13 Simplified Acquisition Procedures. The associated North Industry Classification System (NAICS) is 325414 and the size Standard is 500 employees. This procurement is for the purchase of purified algal toxins (microcystin congeners). See attached Statement of Work. The following FAR clauses apply to this acquisition: 52.212 4, Contract Terms and Conditions-Commercial Items. The following additional FAR clauses which are cited in clauses 52.212 5 are applicable to the acquisition: 52.219-6 Notice of Total Small Business Set-Aside; 52.219-28, Post Award Small Business Program Representation; 52.222-3 Convict Labor; 52.222-41; 52.222-50 Combating Trafficking in Persons; 52.232 33, Payment by Electronic Fund Transfer- System for Award Management. Evaluation Criteria: A. Technical Capability: Demonstrated ability to meet the requirements of the Statement of Work. B. Past Performance: Contractor shall provide three past performance references for work similar to this effort. For each reference identified, include a brief synopsis which includes the date the products were provided, the client for whom the products were provided to (include client ¿s name, telephone number, and e-mail), and a description of the products. C. Price: Contractor shall provide a firm-fixed-price. Price will be evaluated inclusive of options. The government reserves the right to require additional pricing information if necessary to determine that the proposed price is reasonable. D. Basis for Award: Lowest Price Technically Acceptable. All technical questions are to be forwarded through the FedConnect website, under RFQ-RT-16-00026. Questions are due February 17, 2016 by Close of Business (COB). ALL FURTHER INFORMATION RELATED TO THIS SOLICITATION WILL BE LOCATED AT THE FEDCONNECT WEBSITE. Please submit copies of the technical and price proposal to U.S. Environmental Protection Agency, RTP Procurement Operations Division, through the FedConnect website, under RFQ-RT-16-00026. All offers are due by February 24, 2016 at 12:00 p.m. (noon), EST. No telephonic or faxed requests will be honored. E-mail proposals are not acceptable.
