- quote doc and clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. AG-52B1-S-16-0013 is being issued as a Request for Quotation (RFQ), to perform planting services on the Midewin National TallGrass Prairie, Wilmington, IL. The work will be performed in Will County, IL. The work consists of planting approximately 25,000 - 45,000 government furnished plant plugs at specified sites. The North American Industry Classification System (NAICS) code is 115112 and the size standard is $7mil. Contract will begin approximately May 23, 2016 and must be completed by July 1, 2016. BASIS FOR AWARD: will be Best Value. Past performance, Operating Plan strategies; Quality Control Plan and Safety Plan will be factored into the award decision. Past performance and the other factors, when combined, are equal to price or cost. Offerors shall provide both pricing and technical information. Pricing and technical information will be submitted to: Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421. NO FAX QUOTES WILL BE ACCEPTED. E-mailed quotes are preferred at rkmanning@fs.fed.us. Quotes are due by 11:00a.m. e.s.t. March 2, 2016. Service Contract Act wage determination is applicable. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAMS (www.sams.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically at http://orca.bpn. gov., submit only paragraph (b) from the provisions and a DUNS number. (FAR) 52.212-2 Evaluation, Commercial. Clauses and quote documents are attached to this announcement. The information you provide on the Experience Questionnaire, when combined, is equal to price or cost.
