- Combined Synopsis / Solicitation AGAR452.209-70-Alt-1 form SF-1449 form Questions and Answers #1 DOL Wage Determination Experience Questionnaire form Deduction Schedule QASP Quality Assurance Surveillance Plan This is a combined synopsis/solicitation for a commercial service item prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number AG-03R6-S-16-0019 is issued as a Request For Quotation (RFQ). The awarded contract will be a firm fixed price contract. Region One Acquisition serves as the Forest Service contracting office for Western Montana and is located in Building 24 at 24 Fort Missoula Road., Missoula, MT 59840. The Bitterroot National Forest has a requirement for janitorial services for the West Fork Ranger District Administration Building, Log Conference Building, with optional cleaning of the shop and the fire cache bathroom. The West Fork Ranger District is located at 6735 West Fork Road, Darby, Montana 59829, which is 18 miles South of Darby. The Contractor shall provide all management, labor, tools, equipment, bio-based cleaning supplies and incidentals necessary to ensure that custodial services are performed in accordance with the solicitation requirements. The area to be cleaned is approximately 7774 square feet divided into four separate buildings occupying one location. This acquisition is a 100% Small Business set-aside. The applicable NAICS code is 561720 - Janitorial Services. The corresponding size standard is $18 million. Offerors will be evaluated on price, technical capability especially with the utilization of biobased janitorial products, specialized experience, and past performance. The contract will be awarded to the offeror whose submittal, in compliance with the solicitation, is the best value to the Government. The FAR provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to the acquisition. Offers submitted must include past performance information, technical experience information (complete Experience Questionnaire), List of Biobased products, Quality Control Plan, and price (completed Schedule of Items) to be considered responsive. Provide references for similar work during the past three years. In additions, offerors will include with their offer a completed copy of the FAR provision 52.212-3, Offer Representation and Certifications - Commercial Items; proof of active registration in sam.gov. All responsible sources may submit an offer which will be considered by the government. Prospective contracts must be registered in the System for Award Management (sam.gov) prior to award of a contract. Lack of active registration in the SAM database will render an offeror ineligible for award. (SAM replaced the CCR database). Offerors may obtain information on registration and annual confirmation requirements via the internet at:http://www.sam.gov/portal/public/SAM Quotes must be received by February 24, 2016 at 4:30 PM MST.
