Description
The US Department of Veterans Affairs, Boston Health Care System has a requirement for Custom Surgical Packs. The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW). A FAR 13 Blanket Purchase Agreement (BPA) (FFP) or an Indefinite Delivery Indefinite Quantity (IDIQ) (FFP) is under consideration. The Contractor shall provide for each custom pack the following information: (1) the lot number to allow for clear tracking of all packs and products; and (2) an expiration date, and (3) manufacturer of each kit component. The Contractor shall insure that the manufacturer and lot number of each component item is traceable to origin of manufacture and at all intermediate steps. The Contractor shall provide copies of its standard operating procedures (SOP) governing batch control, product tracing, component manufacturing lot numbering, and written policies and/or procedures on component recalls. The Contractor shall ensure that each surgical pack meets Association for Advancement of Medical Instrumentation (AAMI) sterilization process standards, or equivalent standard. This standard must be met and visually stamped on the product. The Contractor is responsible for the sterility and quality of the product. Re-sterilization, re-used or "grey market" products is prohibited. The Contractor shall provide references of work, similar in scope with solicitation information, including Points of Contact, a description of the work completed, and contract numbers. References shall be checked by the Contracting Officer to ensure that your company is capable of performing the Statement of work (SOW). The Government also reserves the right to obtain information for use in the evaluation of past performance from any and all sources. The BPA or IDIQ resulting from the solicitation may have a period of performance from Date of Government Award for up to four (4) additional option years. Multiple awards are under consideration to fulfill this requirement. Offerors who may have an interest in fulfilling this requirement are invited to contact the undersigned Contract Specialist with the following information: (1) Points of Contact, (2) Capabilities Statement, (3) Information regarding their Professional Trade Associations and Accreditations, and (4) Business Size. Interest offeror shall monitor www.fbo.gov for the posting of the solicitation. A SAMPLE Custom Pack Kit is set out below. The US Department of Veterans Affairs welcomes new entrants to Federal Contracting. Service Disabled and Veteran-Owned Small Businesses (SDVOSB and VOSB) contracting with VA are to be registered and certified in VetBiz if seeking consideration under PL 109-461 (www.vetbiz.gov - Vendor Information Pages - VIP). All contractors are required to be registered in the System for Award Management (www.samgov).
