Description
SCOPE OF WORK - Vineland Audiology Hearing Aid Service 1.OVERVIEW This is a sources sought only for a temporary Audiologist to work 24 hours a week for three months at Wilmington VA Medical Center. The purpose of this sources sought is to determine the socio-economic status and number of vendors that are willing and able to provide these services. All responses should include Name, Socio-economic Status, phone number, email address, and capability statement. The Wilmington Delaware VA Medical Center (Wilmington VAMC) is a fully integrated healthcare delivery system providing medical, surgical, nursing home and community care to eligible veterans in a four state area. We are looking to provide audiology hearing aid services to veterans in the southern New Jersey area for one to six months. Services will be provided at either the VA outpatient clinic in Vineland or at the New Jersey Memorial Veterans Home in Vineland. The Audiology Clinic is equipped with a sound booth, clinical audiometer and tympanometer. The facility to be used for the scope of work of this contract will be equipped with hearing aid test box/real ear measurement unit and ear mold modification equipment. 2.PURPOSE The contractor will be required to perform the following tasks:. Hearing aid evaluations, including measurement of UCLs and taking ear impressions.. Entering new orders and service request, certifying repairs, issuing devices, and ordering accessories and batteries in ROES.. Routine hearing aid maintenance and trouble-shooting.. Hearing aid fittings, using verification of gain and output.. Electroacoustic analysis of hearing aid function.. Counseling of patient about hearing and communication options. 3.REFERRALS Referrals will be needed for the following:. Special Audio logic and Vestibular Tests.. ENT referrals. Other disciplines (such as, but not limited to, Speech Pathology, Neurology, OT/PT) Referrals to other services will be entered directly into CPRS, while those needing special audiological testing will be faxed to the Audiology Clinic at 302-633-5540. All charting and consults will be completed electronically in CPRS. Any audiograms taken will be transmitted electronically. Encounters will be entered in either CPRS or QUASAR. 4.CLERICAL SUPPORT Clerical Support will be provided by VAMC Program Support Assistants at the Wilmington VAMC. 5.AUDIOLOGY CONSULTANT QUALIFICATIONS. Current State (any) audiology license.. Must be able to function independently.. Must have malpractice insurance.. Meet or exceed Joint Commission Standards. Must receive mandatory VA training on information security and privacy as well as any other training deemed to be mandatory by administration. Completion of the mandatory trainings may require travel to the Wilmington VAMC. 6.QUALITY CONTROL Every case will be reviewed for completeness of hard copy documentation. Every fifth case will be reviewed for external validity of the audiogram, accuracy of encounter information, electronic transmission of the audiogram and appropriateness of recommendations in light of the consult request. If more than one serious error (directly affects patient care or outcome) or more than five minor errors (do not directly affect patient care or outcome) are found, the contractor will be notified of said problem(s) in writing and must correct such problem(s) within 30 days. If more than two serious errors or ten minor errors are found, the contractor will be deemed to be in breach of contract and may result in contract termination. 7.HOURS/TOUR OF DUTY Services will be provided on a scheduled basis. Contractor will not exceed 24 hours per week and a maximum of 96 hours in a 1 month for a total of 3 Months. 8.PAYMENT FOR SERVICES Payment for services will be per hour based on two exams per hour. Invoices will be provided as requested (if needed).
