Description
STATEMENT OF WORK RADIOPHARMACEUTICALS 1. General Description: The purpose of this contract is for radiopharmaceutical preparations of doses, tagging of blood, testing performed at Contractor facility provided by a board-certified radiopharmacist and delivered to the VA nuclear medicine department at 1111 East End Blvd. Wilkes-Barre, PA. Deliveries will be made Monday through Friday from 7:30am to 4:30pm. Standard daily delivery times will be 7:30am and 9:30am followed by a third delivery as necessary. The fourth delivery of the day will be considered an emergency. Contractor shall pick up day's unused dosages and used vials for proper disposal. Contractor shall provide emergency deliveries as requested by the VA nuclear medicine department. Contractor shall require all of its personnel be licensed in accordance with all applicable laws and regulations prior to the effective date of performance under this Contract. Because of the critical need of these supplies in a timely manner, only vendors who can ensure isotope delivery to medical center within 15 minutes for emergent examinations need to respond to this RFQ. The below radiopharmaceutical products are needed to meet the agency's need, however request for radiopharmaceuticals is not limited to this list. Estimated quantities below are for a period of twelve months. The Contractor shall not be required, except in case of emergency, to furnish such services on a national holiday or during non-service hours as described below. b.Service hours: Monday through Friday, 7:30am - 4:30pm. 6.Delivery Orders: Orders normally will be placed by 2:00pm and delivery shall be required by 7:30am the following work day. Due to the demands of the Medical Center care requirements, there shall be no restriction to the number of orders which may be placed by VA, nor the number of deliveries required per day. Items shall be adequately packaged to prevent damage during handling and storage. The delivery order shall specify: contract item(s) and quantity, time(s) of delivery and applicable VA order number. Orders can be placed orally by telephone and/or fax transmission. 7.Delivery Location: VA MEDICAL CENTER VA Nuclear Medicine Department 1111 East End Blvd. Wilkes-Barre, PA 18711 "Deliveries will be made Monday-Friday (excluding holidays which are listed below) between the hours of 7:30am and 4:30pm, except in case of emergency. "Access to this location during non-business hours shall be determined at the time of the order. 8.Delivery Commitment: Time of delivery specified at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default. 9.Person Authorized to Place Orders: Any Certified Nuclear Medicine Technologist employed by the VA Medical Center, Wilkes-Barre, PA. 10.Unused Doses: Contractor will authorize and detail a return dose program. In FY 2014 approximately 20% of delivered doses went unused. 11.Record Tracking: Contractor will provide an electronic Nuclear Medicine Information System (NMIS) and all relevant upgrades/repairs available to the Nuclear Medicine department for patient dose record tracking. Daily dose administration reports generated by the NMIS will be returned to Contractor. 12.Federal Holidays: Federal Holidays, during which regular deliveries are not required, include: "New Year's Day "Birthday of Martin Luther King, Jr. "Washington's Birthday "Memorial Day "Independent Day "Labor Day "Columbus Day "Veterans Day "Thanksgiving Day "Christmas Day "Any other date so designated by executive order 13.Regulatory Compliance: Contractor shall meet all Nuclear Regulatory Commission, Department of Transportation, F.D.A. and all other agency rules and regulations (Federal and State). Contractor shall perform all quality control procedures as required by Federal and State regulatory agencies. 14.Quality Control: All products offered to the VA must be of the same consistent high quality as would be offered to the general commercial public in the United States. a.Any substitution of product shall have the prior written approval of the VA Nuclear Medicine Service and the Contracting Officer or his/her designee. b.All defective or inferior products shall be replaced within 48 hours at no costs to the VA. c.It is the Contractor's responsibility to provide the VA with recall notices on any product that has been delivered to the VA that may be defective. Notification shall be made to the COR via the most expedient method. 15.Qualifications: The Contractor shall be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals for medical use. 16. Period of Performance: One year, 03-01-2016 to 02-28-2017 -END OF STATEMENT OF WORK-
