Itemized calibration items and services This a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; a written solicitation document will not be issued. The solicitation number is 16-004637. This solicitation is issued as a Request for Quote (RFQ). This is a small business set-aside. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-83. The North American Industrial Classification System (NAICS) code is 811219. Government anticipates this will be a firm-fixed priced contract for one year from date of the award. STATEMENT OF WORK ****SEE ATTACHED FOR ITEMS THAT NEED CALIBRATION AND PREVENTATIVE MAINTENANCE*** The purpose of this Statement Of Work (SOW) is to detail the task and responsibilities (hereinafter, one Vendor) in relation in providing a service agreement that will cover maintenance, preventive maintenance, support, and emergency servicing for Balances, Microscopes, Clinical Pipettes, NIST Thermometers, Stopwatches, Tachometers, and Infrared Thermometers. These Instruments are located in different Sections of the Department of Transfusion Medicine of the Clinical Center at NIH. **The Preventive maintenance (PM) performed on these instruments is required to comply with 21CFR 211.67** **Vendor must be ISO 17025 certified/accredited** **Vendor must be able to complete 100% of the statement of work** Included in this service agreement will be one or more regular preventive maintenance visits, emergency servicing, spare /replacement parts, and support for each listed devices. Work to be performed: Independently, and not as an agent of the government, the contractor will provide all necessary personnel, supplies, and services to perform the work required by this Service agreement as set forth in the sections. The contractor will perform the following: Preventive Maintenance (PM) The contractor will provide scheduled preventive maintenance once/or several times, depending on the type of equipment, in the 12-month period. The PM will be scheduled by a qualified service representative and days in advance of the scheduled due date. Services will be performed on the following: 1. Balances: Semiannual maintenance- Calibration / Certification - Standards Information - Final Status 2. Microscopes: Annual maintenance - Cleaning (Base-feet-stage plate-optical surface-set screw and knobs- lampcover-objective lens-power cord for contamination-lubrication) Functional Test: Inspection of Base - feet-stage plate-zoom control-magnification lens-adjustment knobs-light bulb-light switches-power cords. 3. Pipettes: Semiannual Calibration - As Found/As left data - Accuracy % - Standards used Information - Depending on the type of pipettes up to 10 check points - Certification - Final Status. 4. NIST Thermometers - Annual Calibration - standards-as found-min./Max.- Final-certification 5. Tachometer Annual service - Standards used - Revolution per minute (optical) - As found - Min./Max./Final-Certification 6. Stopwatch: Annual calibration - Standard used - As found - Accuracy - Final - Certification Adjust or repair any item that does not meet manufacture's qualification criteria. Provide documentation of all work to the DTM. Parts /Supplies The contractor will supply all parts and supplies needed for service and maintenance; otherwise, a separate quote will be submitted. Period of Performance Service will be performed MON-FRI 8am to 5 pm. Labor and Travel The contractor will provide all labor and travel costs will be listed on the quote. The contractor will provide full service coverage and telephone support to correct all equipment malfunctions. CAPABILITY STATEMENT - Any potential Manufacturer must demonstrate the following: 1) The offeror shall submit product literature for the supplies it is proposing. Capability Statement(s) should clearly identify the following: a. Name of the Manufacturer, quantity and location where the units are produced (domestic and/or international); b. A signed statement to state, "We are the Manufacturer of the items and/or we are authorized distributors of company X"; c. Estimated Number of days, after receipt of order that is typical for delivery of such items; d. Any other relevant information that is not listed above which the Government should consider in developing its minimum specifications and finalizing its market research. 2) Provide the following business information: a. DUNS number b. Company Name c. Company Address (including point of contact, phone number and email address) d. Type/Size of company, NAICS code as validated in SAMS. QUESTIONS DEADLINE: All question requests for NIH are to be submitted via email to Milton.Dartoozos@nih.gov by February 24, 2016, 10:00 a.m. EST. QUOTATIONS DUE: All quotations are due, no later than, 12:00 pm, EST on March 1, 2016 email to Milton.Dartoozos@nih.gov. The solicitation does not commit the Government to pay any cost for the preparation and submission of a quote or proposal. PROVISIONS and CLAUSES: The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this solicitation. The following agenda has been attached to this provision: see above. Certifications Commercial Items FAR 52.212-3 Offeror Representations and Certifications-Commercial Items. (APR 2012). The clause at FAR 52.212-4, Contract Terms and Conditions, The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders, Commercial Items applies to this acquisition. The following FAR clauses cited are applicable: FAR 52.203-13, FAR 52.209-6, FAR 52.211-6, FAR 52.217-5, FAR 52.217-8, FAR 52.217-9, FAR 52.222-3, FAR 52.222-19, FAR 52.222-21, FAR 52.222-56, FAR 52.222-35, FAR 52.222-36, FAR 52.222-37, FAR 52.223-15, FAR 52.223-18, FAR 52.225-1 and FAR 52.232-33, FAR 52.246-16.
