- *****BEGIN FBO DESCRIPTION ***** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA249-16-Q-0267 and the solicitation is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86-2/01/01/2015. (iv) This requirement is unrestricted. (v) CLIN0001 - Twelve (12) Boxes of Ten (10) each Vascular Closure Devices, Collagen Plug (Brand Name or Equal to Angio-Seal). (vi) See the attached 1449 CLINS for more information of the description of CLIN 0001 above. (vii) The date of delivery is ARO+10. The place of delivery is 1030 Jefferson Avenue Attn: 111, Tiensha Salter, Memphis, Tennessee 38104. FOB is Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (ix) The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i)Acceptability of equal to brand name (Angio-Seal) of the item offered to meet the Government requirement; (ii)Price; (iii)Past performance (see Far 15.304); Technical and past performance, when combined are of equal importance to price. (x) The provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, shall be on file at www.sam.gov at the time of offer. (xi) The clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. (xii) The clause at 52.212-5, Contract Terms and Conditions Required to implement Statues Or Executive Orders - Commercial Items, applies to this acquisition the following additional FAR clauses cited in the clause are applicable to the acquisition: (b) 22, 25, 26, 27, 28, 29, 30, 33, 40, 41, and 50. (xiii) All customary warranties, guarantees, and assurances shall apply to this requirement.
