- Additional Terms and Conditions_SDPI Support Services (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 16-236-SOL-00006 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86. (iv) The North American Industry Classification System (NAICS) code is 541990 with a size standard of $15M. The Indian Health Service (IHS), Division of Acquisition Policy intends to solicit a firm fixed priced purchase order to Small Businesses only. (v) The Contract Line Items (CLINs) and pricing structure: *See the attached Additional Terms and Conditions, Section B - Products or Services and Prices/Costs (vi) The contractor will work under the direction of the IHS Division of Diabetes Treatment and Prevention (DDTP) and the Portland Area federal leadership in accomplishing the following tasks: 1. Diabetes Surveillance; 2. Technical Assistance to SDPI grantees in the Portland Area; 3. Education and Training; and 4. Area-wide Outreach Activities. (vii) The delivery and acceptance term for this order is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Oct 2015), applies to this acquisition and a statement regarding any addenda to the provision. (ix) 52.212-2, Evaluation - Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Capability; Key Personnel and Management Approach; Relevant Past Performance; and Price. See the attached Additional Terms and Conditions, Section M - Evaluation Factors for Award. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items (Nov 2015), with its offeror. (xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (May 2015) applies to this acquisition. Addenda to the clause is included in the attached Additional Terms and Conditions. Questions are due on February 26, 2016. Vendors must confirm IHS's receipt of their quotation. (xvi) Interested vendors capable of furnishing the government with the item specified in this combined synopsis/solicitation should submit their quotation to the below email address. The quotation must reference Solicitation number 16-236-SOL-00006. All responsible sources may submit a quotation, which if timely received, will be considered by the agency.
