- Past Performance Questionnaire (PPQ) (see FAR 52.212-1 (b)(10) (3 Pages) Attachment 1 - SCA DOL Wage Determination Number 2005-2095, Revision 17, dated 12/29/2015 (10 Pages) Solicitation - Request for Proposal (RFP) Number HSCG84-16-R-AA5499 (61 Pages) Please be sure to Read the Entire Solicitation and complete all required documentation when submitting your proposal. A Firm-Fixed contract will be awarded to the responsible proposal, whose proposal conforming to the solicitation will be most advantageous to the government, price, past performance and technical factors considered. All services shall be in strict adherence to all service requirements in accordance with a Performance Work Statement (PWS), as well as applicable local, state and federal regulations, Coast Guard Facility Instructions and shall be consistent with current industry practices. The applicable Department of Labor Wage Determination is No. 2005-2095, Revision No. 17, Date of Revision 12/29/2015; and will be included in the solicitation. Contractors must submit the following documentation along with their proposal: Corporate Experience/Management Capability; Licenses and/or certifications required by the State of Virginia; Certificate of Insurance as required by FAR 52.228-5 and Past Performance information in accordance to the requirements set forth by FAR 52.212-1 (b)(10). No paper copies of the solicitation will be distributed. All vendors will be responsible for obtaining the solicitation and any subsequent amendments from the www.fbo.gov website. CURRENT CONTRACT INFORMATION: The proposed acquisition is a follow-on requirement of contract HSCG84-11-N-AA5376, which was awarded/effective 01 May 2011 to Petrus Environmental Services, Inc., Roanoke, VA and expires 30 April 2016, for a total value of $210,650.40 that included a Base Year and Four Option Years (60 months) from 01 May 2011 through 30 April 2016. All responsible sources may submit an offer, which will be considered by this agency. If your offer is incomplete it may not be considered.
