Attachment 2 Questions for Industry Attachment 1 Draft Performance Work Statement for Patient Safety Program Support (83 pages) This is NOT A REQUEST FOR PROPOSALS OR a Request for Quotes ; it is strictly a request for Sources Sought. Neither unsolicited proposals nor any other kind of offer will be considered in response to this Sources Sought. No contract will be awarded pursuant to this announcement. 1. SUBJECT: The 773 Enterprise Sourcing Squadron (773 rd ESS/PK) is conducting market research to obtain information regarding: (1) the availability and capability of qualified business sources; (2) whether they are small businesses, HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses, or small disadvantaged businesses; (3) their size classification relative to North American Industry Classification System (NAICS) code 541611, Medical Office Management Consulting Services or Consultants; (4) review of the Performance Work Statement for Patient Safety Program Support and (5) answers of questions to industry. 2. DISCLAIMER: This posting is issued solely for conducting market research, gathering information and planning purposes to determine qualified, experienced and interested potential sources and does not constitute a solicitation. Neither unsolicited proposals nor any other kind of offers will be considered in response to this posting. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Respondents are solely responsible for all expenses associated with responding to this posting. All information received in response to this posting marked as "proprietary" will be handled accordingly. Information provided as a response to the posting will not be returned. The 773 ESS/PK will not entertain any questions concerning the composition, requirements, or the nature of services to be performed under any future request for proposal (RFP). 3. BACKGROUND: The Air Force Medical Service's (AFMS) commitment to delivering quality, evidence-based, safe care cannot be overstated. This requires on-going culture change throughout the enterprise, ultimately resulting in the AFMS becoming a High Reliability Organization (HRO). It is the AF's intent to accomplish these goals through the execution of a contract encompassing patient safety personnel at each MTF coupled with training materials keyed to foster a workplace-based safety culture transformation, guided by experts in patient safety at key headquarters positions (AFMOA, Air Mobility Command (AMC), and Air Combat Command (ACC)). 4. REQUIREMENTS: Please review the attached Performance Work Statement (PWS) to determine the entire need of the Air Force's required services. This requirement falls under NAICS Code 541611, Medical Office Management Consulting Services or Consultants. Any questions must be e-mailed no later than 09 March 2016 by 2:00 PM Central Standard Time (CST). Please provide whether you would be a Prime Contractor or a Sub Contractor for this effort. Also include the business size status (i.e. 8(a) Business Development Small Business Certification, Historically Underutilized Business Zone (HUBZone, etc.) under NAICS 541611 in your responses, also indicating if you are a GSA contract holder and if the requested service is on your respective GSA contract. This posting is not a solicitation. The Government will not pay for any information submitted, or for any costs associated with providing the information. Please include a point of contact, phone number, e-mail address, website information, and indicate whether the company is a foreign or domestic entity in the contents of the e-mail. Indicate whether large or small business entity, provide DUNS number, CAGE Code and SAMS expiration date. The response should describe the company's capability to offer on an "All", "Partial with subcontracting", or "None" based on the Performance Work Statement (PWS). All Responses to this Sources Sought (posting) are due by 11:00 AM CST on 28 March 2016. LATE SUBMISSIONS: Failure to respond to this posting does not preclude participation in any possible future competition, nor will information provided in response to this POSTING be used to exclude anyone from responding to any future requests for proposals. Communications with 773 ESS/PK with regard to this posting will only be permitted in writing (via fax or email) during the posting response period. Responses to this posting received by 773 ESS/PK after the submittal deadline and time indicated may be considered. The Respondent assumes the risk of the method of dispatch chosen. Postmarking by the submittal date and time shall not substitute for actual response receipt. 7. OWNERSHIP OF RESPONSE TO POSTING: All informational material submitted in response to this posting becomes the property of 773 ESS/PK. 8. RELEASE OF CLAIMS, LIABILITY, AND PREPARATION EXPENSES: Under no circumstances shall 773 ESS/PK be responsible for any response preparation expenses, submission costs, or any other expenses, costs, or damages, of whatever nature incurred as a result of the respondent's participation in this posting process. Respondent understands and agrees that it submits its response at its own risk and expense, and releases 773 ESS/PK from any claim for damages or other liability arising out of the posting process. 9.
