- The Department of Veterans Affairs, VA Providence Healthcare System, has a requirement for a Pharmacist for research purposes. A fixed price contract for approximately 5 years is anticipated and shall be awarded. It is the Government's belief that only a certain individual with clinical, managerial, and experience on the current research projects possess the required capabilities to successfully meet this requirement. The Government will proceed with a sole-source award as planned. Inquiries/information received after the established deadline (March 10, 2016) shall not be considered. The submission of any data for review shall not impede award of this contract as planned. The Government does not intend to pay for information solicited. This synopsis is issued pursuant to FAR 6.302-1. STATEMENT OF WORK TITLE OF PROJECTS AND PROJECT ID NUMBER: 1.Title: "Group Medical Visits in Heart Failure for Post-Hospitalization Follow-Up" Background: The primary goal of this research is to improve the quality of care and outcomes for patients discharged with heart failure using an innovative multidisciplinary group intervention approach. Heart failure is a complex chronic illness where comprehensive patient-centered care is difficult and resource intensive. One potential solution is to use shared medical appointments (SMA), where a multi-disciplinary team of providers with expertise in nutrition, nursing, behavior and medication management join to manage heart failure in addition to and support of, the patient's regular physician visits. We will enroll patients from the Providence VA Hospital who were within 12 weeks of discharge from a heart failure hospitalization and randomized them to receive SMA intervention or usual care. Results from this project are expected to add an innovative intervention that could improve outcomes important for both the patient and the VA: health status, hospitalizations, and mortality. The purpose and scope of this research contract is for the contractor to provide clinical research services in shared medical appointment for heart failure. This research project has been reviewed and funded by VA Health Services Research and Development Service. The project number is IIR 14-293. 2.The contractor shall work with the VA Principal Investigator to carry out the services/requirements described below. Requirements: The purpose and scope of this research contract is for the contractor to provide clinical research services in shared medical appointments for heart failure. The work responsibilities requested are budgeted in the VA HSR&D Merit Review grant: Group Medical Visits in Heart Failure for Post-Hospitalization Follow-Up. The services required are to provide clinical research services in shared medical appointment (SMA) for heart failure and management of the pharmacotherapy issues for the Shared Medical group. Assessment of the medication regimens and medical records of the participants in the Shared Medical Group to include recommendations regarding appropriate drug therapy, monitoring parameters, potential interactions or duplicate therapy, problems with compliance, possible strategies to simplify medication regimens and healthcare economic considerations. The first year services will consist of participation in and overseeing and supporting the SMA intervention teams and management of the pharmacotherapy issues as described, in addition to conducting focus groups and fidelity assessment of the SMA for qualitative data collection. In Years 2 and beyond, the contractor will continue to conduct SMA in heart failure and management of the pharmacotherapy issues, conduct fidelity assessment and focus groups for qualitative data collection of the SMA, as well as continued guidance to the research team. 3.Delivery: Services in support of delineated requirements will be performed from the effective date of the contract, for up to 5 years. 4.Progress and Compliance: Progress and compliance will be monitored by the Principal Investigator. Findings will be reported to the Contracting Officer Representative (COR). 5.The contractor shall submit invoices in a timely manner to the VA Principal Investigator for approval for services provided under this contract. All invoices are subject to approval by the Contracting Officer Representative (COR). 6.Performance under this contract is subject to all VA and VHA regulations and policy without limitation as to those now in effect or as may be established and is subject to oversight of the VA Principal Investigator in consultation with the COR. 7.The research under which this project is funded may be extended, supplemented or expanded by additional funding from the VA's Office of Research and Development based on congressional appropriations.
