This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit offers. Solicitation VA249-16-Q-0296 is issued as a Request for Quotes (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. This procurement will be a service disabled small business set-aside. NAICS Code is 561720 and size standard is $18.0 million. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all parts, service, manuals and tools as specified in the attached statement of work. This service is required for the Nashville VAMC 1310 24th Avenue South Nashville, TN 37212, Murfreesboro VAMC 3400 Lebanon Pike Murfreesboro TN 37129, and Chattanooga VA Outpatient Clinic 6098 Debra Road Ste 5200 Bldg 6200 Chattanooga TN 37411 within VISN 9, in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a quote on all itemized items listed in the price schedule for each unit of pricing. Site Survey: There will be a site survey on Wednesday March 9, 2016. Nashville's VAMC site survey will begin at 10:00 AM. Please contact Sean Stephens at 615-225-4510. Murfreesboro VAMC site visit will be begin at 1:00 PM. Please contact Kathleen Hoffman at 615-225-4510. Chattanooga will not have a site survey location. Contractors will bring sample devices and odor neutralizing fragrance to show the COR the site survey. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, overall total price for services, and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document.
