Description
White Lab Coat Rental, Laundry and Maintenance Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, using FAR Part 13 Simplified acquisition procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is CSOSA16Q0010 and is issued as a Request for Quote (RFQ). The Court Services and Offender Supervision Agency (CSOSA) intends to acquire Lab Coat Rental Services. This procurement will be a 100% Small Business Set-A-Side. The solicitation is set aside for small businesses that fall under NAICS Codes 812331/812320. The size standard is $5.0 Million for companies under NAICS Code 812320. The size standard is $30.0 Million for companies under NAICS Code 812331. Responses shall be in accordance with the RFQ and the Statement of Work, CLIN 0001 thru 4002. The Period of Performance will be One Base Year of Twelve (12) Months plus Four (4) One Year Option Periods beginning on or about April 16, 2016 through April 15, 2021. Delivery of Services will be to the designated addresses in the RFQ. The following provisions are applicable to this acquisition: FAR 52.212-1 Instructions to Offerors - Commercial Items; FAR 52.212-3 Offeror Representations and Certifications - Commercial Items; FAR 52.212-4 Contract Terms and Conditions -- Commercial Items; and 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (note: as of 1 January 2005, offerors are required to enter their representations and certifications on line at http://federalcontractregistery.com); FAR 52.212-2 Evaluation - Commercial Items. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86-2 (02-01-16). Contract evaluation and subsequent award shall be based on the Lowest Priced Technically Acceptable (i.e. Lowest Price Technically Acceptable) proposal that is determined to be most beneficial to the Government Responses to this Combined Synopsis/Solicitation must be signed, dated, and received via electronic mail by March 24, 2015 no later than 12:00PM Eastern Time at The Court Services and Offender Supervision Agency (CSOSA), Office of Procurement, 633 Indiana Ave., NW Washington DC 20004. Responses should be marked with the solicitation number and addressed to Lawrence C. Durden, Contract Specialist. Vendors not registered in the Systems For Awards Management (SAM) database prior to award will not be considered. Vendors may register with SAM by calling 1-800-985-7658 or register on line at http://www.SAM.gov No information, questions concerning this RFQ or request for clarifications will be provided in response to telephone calls. Formal communications such as requests for clarification, information or questions concerning this RFQ shall be submitted in writing via email only to: Lawrence C. Durden, Contract Specialist, at Lawrence.Durden@csosa.gov. INSTRUCTIONS, AND NOTICES TO OFFERORS Special Instructions Offerors shall provide a firm fixed price quote for the Rental, Maintenance and Cleaning Service for the Button Front - White Lab Coats identified in the Statement of Work. The price shall be inclusive of the supply of all coats identified in Statement of Work, weekly cleaning service for the coats with weekly pick-up and drop-off at the various CSOSA locations listed in the SOW. Cleaning shall include pressing and hanging of coats on hangers and shall also include minor repair such as small tears and button replacement. Vendor shall supply a bin for soiled coats. Weekly cleaning shall be provided for coats identified in attachment 1 as needed, assuming an average of 156 clean Lab coats per week over the period of performance. The first delivery of lab coats shall take place no later than April 18, 2016. Coats shall be white and shall be the sizes indicated in the attached list. Each coat shall have a label attached to the inside collar with the last name of the individual as indicated in the attached list. Coats shall be polyester/cotton, knee high length, long sleeve with front buttons. 1. The Contractor shall consider environmentally preferable products or services that have a reduced impact on the environment in performance of this effort. 2. Please include references for 3 recent and relevant contracts or clients. Provide client names, contact names and phone numbers. 3. Please include your DUNS number with your quote. 4. Offerors shall submit quotes via e-mail to Lawrence.Durden@csosa.gov. Quotes received after the required date and time will not be considered for award. 5. Offerors must be registered in the System for Award Management (SAM) by the date specified for receipt of offers. Information on the SAM can be found in FAR 52.204-13 - Instructions to Offers - Commercial Items. (OCT 2015) Offerors Questions and the Government's Answers (a) Questions concerning the solicitation document shall be submitted via email to Lawrence.Durden@csosa.gov. All questions must be received no later than 2:00 P.M. on 7 March 2016. All questions will be answered on or about 10 March 2016. Offerors are responsible for following up for any posted changes to the solicitation via amendment at the Federal Business Opportunities website (www.fbo.gov). (b) In order to receive responses to questions, Offerors shall cite the solicitation section, paragraph number, and page number. The CSOSA recommends that Offerors ensure the question is written to enable a clear understanding as to the Offeror's issues or concerns within the referenced paragraph. Statements expressing opinions, sentiments, or conjectures are not considered valid inquiries or comments for this purpose and will NOT receive a response from CSOSA. Further, offerors are reminded that CSOSA will NOT address hypothetical or theoretical questions aimed toward receiving a potential "evaluation" decision from CSOSA. Answers to questions will give due regard to the proper protection of proprietary information. Quote Submission (a) The Quote shall be submitted via email. EST March 24, 2015 to: Court Services and Offender Supervision Agency Office of Procurement Attention: Lawrence Durden Lawrence.Durden@CSOSA.gov BASIS FOR AWARD In accordance with FAR 13.106-1 an award will be made to the contractor that can provide the Lowest Price Technically Acceptable (LPTA) quote. The evaluation will be conducted in accordance with Federal Acquisition Regulation Part 13.106-2. Award will be made on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-cost factors. The Request for Information was posted to FBO.gov on The following will be considered when determining ability to meet all of the requirements: An established capability to pick up and drop off lab coats in the Washington, D.C. region. Evaluation factors other than price when combined are approximately equal to price. Notice of Award A written notice of award or acceptance of an offer electronically mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer, whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. It is the sole responsibility of the offeror to review the email for any amendments or updates.
