Description
Sole Source Notice (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 16-236-SOL-00026 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85. (iv) The North American Industry Classification System (NAICS) code is 541519 with a size standard of $27.5M. The Indian Health Service (IHS), Division of Acquisition Policy intends to award a firm fixed priced, 3 months bridge purchase order as a result of this RFQ. (v) The Contract Line Items (CLINs) and pricing structure: CLIN0001: 3 months IHS Web-based Environmental Health Reporting System support services. Period of Performance: 10 March 2016 thru 9 June 2016. (vi) This bridge order is to acquire 3 months support services for the IHS Web-based Environmental Health Reporting System. A follow-on contract is to be competed in near future. The system will be utilized at the national, regional, and field levels by approximately 300 Indian Health Service and Tribal environmental health specialists. (vii) The delivery and acceptance term for this order is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Oct 2015), applies to this acquisition and a statement regarding any addenda to the provision. (ix) 52.212-2, Evaluation - Commercial Items (Oct 2014) (a) The Government will award a contract in accordance with the current terms and conditions of the incumbent contract (HHSI236201000002I), which was awarded upon a best value evaluation process with adequate competition basis. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items (Nov 2015), with its offeror. (xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (May 2015) applies to this acquisition. Addenda to the clause is included in the attached Additional Terms and Conditions. (xii) 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Feb 2016) (a) The Contractor shall comply with the following Federal Acquisition Regulation (FAR) clauses, which are incorporated in this contract by reference, to implement provisions of law or Executive orders applicable to acquisitions of commercial items: (1) 52.209-10, Prohibition on Contracting with Inverted Domestic Corporations (Nov 2015) (2) 52.233-3, Protest After Award (AUG 1996) (31 U.S.C. 3553).
