Description
PLEASE VIEW ATTACHED DOCUMENTS OUTLINING REQUIREMENTS Subcontractor shall provide the following: SCOPE OF WORK: To maintain, service and inspect all Refrigerators, walk-in coolers, heating and air conditioning units to assure that they are working properly at all times. Vendors must provide filters for all units. Vendor must respond within 24 hours to all emergency service calls or give a detailed explanation as to why not. Vendors will provide preventive maintenance service 4 times a year or as needed. To provide mechanical services to the coils, compressors, and condensers as listed below: The Coils Clean all evaporator coils at the beginning of cooling season. Clean drain pans once every three months or as necessary. Change air filters and belts as needed, vendor must provide filters and belts. Adjust fan motors to manufactures specifications for optimum performance. Check the amperage draw on the fan motors regularly during inspections. Inspect the wiring and control for any burns. Grease and oil all bearings and shafts. The Compressors Set all operating controls to get the highest capacity from the machine. Check and charge all systems with refrigerant as needed. Adjust all valves accordingly. Set the stages of each compressor to run efficiently. Check contactors and line starters. PROPOSAL SUBMISSION: This is a Subcontracting opportunity. It is not a contract with the United States Department of Labor. However, All contract actions are subject to the approval of the United States Department of Labor in accordance with Federal regulatory procedures and mandates of the Federal register. THE SUBCONTRACTOR MUST: Must be in compliance with all local, state and federal regulations and guidelines •ü Work to be accomplish in safe and efficient manner with minimal or no disruption or endangerment to trainees and staff. •ü Must provide a copy of current license and insurance. •ü Provide to the center all warranties, guarantees, equipment and any other information related to the above project. •ü Provide all labor and material to complete the work and details mentioned above. •ü The above work should be completed within appropriate codes, ordinance and regulations enforced by the city of New York, State of New York and other relevant Federal Agencies including OSHA and the FAR. •ü Provide copies of payrolls (Service Contract Act) •ü Must be registered on the SAM.gov website •ü Must complete the Vendor Packet which consist of the following items. 1. New vendor form, fill out the entire form. DUNS section is a MUST 2. Classification Form 3. W-9 form 4. Non conflict of Interest 5. Terms and Condition 6. Anti-bribery form-Sign and approve •ü The subcontractor shall submit to the center all warranties, guarantees, equipment literature and any other information that relates to this project. •ü No additional work shall be performed without the expressed approval of a written change notice to this subcontract. Performance period: The services should shall be in effect April 1 st, 2016- March 31, 2017. Successive renewal of this contract may be made subsequent to annual solicitation of services following the first year. Additional work may be requested from time to time, which should be authorized by written change to the resultant agreement. SAFETY AND HEALTH: All work shall be accomplished in a safe and efficient manner, with minimal disruption to the operation of the Academy. The subcontractor represents, by acceptance of any resultant agreement, that they are in compliance with all regulations and standards flowing from the Occupational Safety and Health Act of 1970 ( PO 91-596, 29 USC 651 et seq) and further that in the event hazardous materials are involved, will comply with Federal Standards No. 310 dated July 1971 to the latest change, entitled "Symbols for Packages and Containers for Hazardous Chemicals and Materials VENDOR ACKNOWLEDGEMENT: Vendor acknowledges that by acceptance of any resultant agreement, all work and material used in the performance of the contract duties meet federal, state, local and OSHA regulations Vendor will also acknowledge by signature that the specifications, terms, conditions, and instructions set forth herein are acceptable and that if selected they are willing and legally able to begin performance of services on date service contract is approved Submit bids by mail or email to: Brooklyn Job Corps Center ATTN: Procurement Office 585 DeKalb Ave. Brooklyn NY 11205 Mcnish.katiuska@jobcorps.org Vargas.rosalba@jobcorps.org BID SHEET In compliance with the Bid Provision, Scope of Work, Terms & Conditions, and other specifications contained in this RFQ, the undersigned hereby proposes to provide the services and materials listed for the center. The Rates herein are no higher than would be for any other customer, either commercial and/or agencies of the United States Government, for similar services. Services from April 2016-March 31 st, 2017 Monthly Quarterly Yearly Include a detailed outline proposal on vendor letterhead; Above rates above should also include necessary transportation, and any supervision required to accomplish Scope of Work.
