Description
THIS IS NOT A SOLICITATION FOR PROPOSALS. THIS IS A SOURCES SOUGHT SYNOPSIS ONLY This Request for information (RFI) is for the purpose of identifying contractors that possess the capabilities outlined below. The NAICS code for this RFI is 621111 and the Product Service Code is Q201 The Bureau of Reclamation is required to provide medical services for their employees and workplaces. Medical examinations including medical surveillance, preventive medical services, and clearance examinations to meet regulatory requirements may be needed based on employee job descriptions and job tasks. Required services are based on regulatory requirements including Office of Safety and Health Administration (OSHA), Department of Transportation (DOT), Department of Motor Vehicles (DMV), or specialty examinations based on Reclamation Safety and Health policies and standards (www.usbr.gov/ssle/safety/directives.html ). Preventive services are typically based on potential exposures due to the nature of the work, such as rabies or Hepatitis A vaccines. Reclamation uses the guidelines of the Department of the Interior Occupational Medicine Handbook (https://safetynet.doi.gov/information/general/medical/DOI_Occup_Med_Prog_Handbook_Dec2010.pdf ). The locations of the services needed are throughout the 17 western states which includes Arizona, California, Colorado, Idaho, Kansas, Montana, Nebraska, Nevada, New Mexico, North Dakota, Oklahoma, Oregon, South Dakota, Texas, Utah, Washington, and Wyoming. The services may be accomplished through a provider network. The types of services include but are not limited to: 1.Pre-placement, baseline, periodic, and exit examinations 2.Regulated Exams (OSHA, DOT) 3.Specialty Exams for Crane Operators, Hazardous Waste Workers, Respirator Clearance, Asbestos, Lead Work, Tower Climbing 4.Fitness for duty examinations and consultation 5.Accommodation placement examinations and consultation 6.Physician Consultations 7.Clinical Laboratory Sample collection and test results. 8.History/Review of Medical Tests 9.Medical Imaging/Radiology 10.Ancillary Diagnostic Services (Audiograms, Cardiac Stress Tests, EKGs, Spirometry, Visual Screening,) 11.Audiograms and audiometric history comparison to support Hearing Loss Prevention Program. 12.Musculoskeletal Evaluation (back, neck, shoulder, hand/wrist, lower extremity assessment) 13.Immunizations and Vaccinations (Flu Shots, Rabies, Tetanus, Hepatitis A and B) This RFI shall not be considered as a request for proposal or as an obligation on the part of the Government to acquire any products or services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor submission of this RFI or the Government's use of such information. The Government reserves the right to reject, in whole or in part, any contractor's input resulting from this RFI. No contract will be awarded as a result of this announcement. Data submitted in response to this RFI will not be returned. INSTRUCTION FOR RFI SUBMISSION: Interested parties may submit a response in an electronic format only via email to jchamberlin@usbr.gov no later than 28 March 2016 2:00 pm MDT. Electronic files should be in Microsoft Office Word or PDF format. All information should be UNCLASSIFIED material only. Respondents should include as part of their submission: a.A synopsis of the company's capabilities to provide qualified personnel to provide the services throughout the 17 western states. b. DUNS Number c. Company Name d.Company Address e.Business size, SDVOSB, HUBZone, or Woman owned status, as validated via the System for Award Administration (SAM). All offers must register on the SAM located at http://www.SAM.gov f.Company point of contact, phone and email address. Any company proprietary information must be marked as such, the RFI information should not exceed a total of 30 one-sided 8 1/2x11 pages, with one inch margins, and font no smaller than 12 point.
