Description
1)GENERAL INFORMATION A.Title of Project: Forum Data Management Procurement B.Background: The Lebanon VAMC has a need for a Forum Data Management System in the Optometry/Ophthalmology Clinic. 2)SCOPE OF WORK The Forum Data Management System will be utilized by Optometry and Ophthalmology Staff to enable data retrieved via Visual Field, and Cirrus OCT machines to be stored on a secure site and exported to the patients chart immediately. Currently the data must be printed and scanned into the patient chart which is not efficient, secure or real time. The Forum system will also allow patients cared for at satellite facilities to have their data exported to their chart as well, and will also enable them to be followed at their local site rather than come to Lebanon for their cataract and/or Glaucoma eye care. "The unit purchased shall include: oPACS system must be able to create Consults or Orders from CPRS or VISTA and transfer the patient demographic data into its system with VA VISTA being specified as the Issuer of Patient ID. No manual entering of patient demographic data is required at the PACS System or on the DICOM capable instrument. oMust be FDA 510K approved and listed as a Medical Device. oPACS System must be able to send the Consult or Order to the instruments DICOM modality worklist. oPACS System must connect any brand of non-DICOM capable diagnostic instrument with an export capability, and label the device type and laterality in the PACS system. oPACS System must be able to automatically send images and reports from the instrument to its system via a DICOM gateway. oPACS System must not require the use of an external PC or import computer for DICOM capable instrument modalities. oPACS system must be able to store and access raw instrument data from Zeiss Field Analyzer, Zeiss Cirrus HD-OCT, Zeiss Visucam Fundus Camera and Zeiss FF450/Visupac Fundus Camera systems. oPACS system must be able to download all past raw data from one or more Zeiss Field Analyzers, Zeiss Cirrus HD-OCT's, Zeiss Visucam Fundus Camera and Zeiss FF450/Visupac Fundus Cameras acquired within the main clinic and all satellite clinics upon installation of the PACS system. oPACS system must create in its viewer all past visual field test reports in color from one or more Zeiss Field Analyzers located in the main clinic and all satellite clinics at the time of installation. oPACS system must be able to display Zeiss Field Analyzer Glaucoma Progression Analysis (GPA) and enable the clinician to manipulate data and store baseline changes at the workstation. oPACS system must be able to present VFI Index, Mean Deviation, Pattern Standard Deviation and Global Indices as a glance summary of all past and present tests. o PACS system must be able to display all Cirrus HD-OCT past and present test data from one or more Cirrus instruments located in one or more VA clinics. This date must be accessible to enable viewing and manipulation at the workstation. oPACS system must be able to display and allow the clinician to scroll through the various Cirrus Macula Cube B-scan slices. oPACS system must be able to download at time of installation all past Zeiss Visucam Fundus Camera and Visupac Fundus Camera photographic images. oPACS system must be able to automatically label images and reports coming from the instruments to enable the clinician to sort test data by visit date/s, modality type/s, right eye, left eye, or both eyes. oPACS system must be able to display images in pre-formatted clinical display groupings for Glaucoma, Retina, and Cataract cases. oPACS system viewer must be able to display images in a right eye/left eye display format. oPACS system must be able to tag individual test reports/images as Favorites to enable easy access on future patient visits. oPACS system must also be able to transmit DICOM images to other Zeiss modalities that are procured by the Lebanon VAHCS. (US) - Includes 3 user licenses/ea Qty - 1 *000000-2133-315 Zeiss OS/License Zeiss Regina Workplace User - Volume pack 25X/ea (qty 1)266002-1150-893 "Contractor shall provide all hardware required for the Forum Data Management System (i.e. server(s), clients, UPS, etc ¦) "This procurement will require sole source or name brand status, as the only equipment which will be compatible with the Zeiss equipment; Cirrus OCT, and Visual Field Machines is the Zeiss Forum Data Management System. 3)INSTALLATION Contractor shall provide installation and project management services for this procurement. Contractor shall ensure all hardware is fully functional and capable of the Forum software and coordinate with clinical staff and Biomedical Engineering to physically install the hardware and ensure it is operational at the installed location. Contractor shall install applicable software on all equipment, clients and server(s) and ensure the application is running correctly. Contractor will configure the connections on all equipment to ensure communication between all equipment, clients and server(s) is correct. Contractor shall perform a full functionality test of each piece of equipment to ensure that the equipment is properly sending data either directly to Forum or to its client PC and then to Forum. Contractor will also test data transmission between the Forum solution to VistA Imaging. Contractor will also ensure that all data transmission is bi-directional, allowing information from VistA to be pulled into Forum and then to the equipment and client PCs. Biomedical Engineering will receive all back up discs, licensure, software keys and passwords necessary to fully maintain the Forum solution. 4)CLINICAL TRAINING On-site go-live clinical training shall be provided at Lebanon VA Medical Center and at applicable CBOCs where the installation is taking place. Sessions will be scheduled after the Forum solution has been installed, fully tested and determined ready for use. Sessions will be scheduled at the convenience of clinical staff. 5)TECHNICAL TRAINING On-site go-live technical training shall be provided at the Lebanon VA Medical Center. Sessions will be scheduled after the Forum solution has been installed, fully tested and determined ready for use. If there is off-site technical training at a Zeiss facility, please describe the training offered, prerequisites required to take the training and please include as an option in the proposal. 6)WARRANTY All equipment, software and hardware shall have a minimum of one year warranty. Remote and on-site support will be provided at no charge to the Lebanon VA Medical Center during the warranty period. Installation, Training, and Licensures are included in this procurement, and are indicated above, on the Equipment Request, and the Evaluation Criteria. 7)DELIVERY Delivery shall be within thirty (30) days from the time of award. The Zeiss Forum and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment in to the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through the Contracting Officer Representative. The North American Industry Classification System (NAICS) Code for this acquisition is 339113 Size Standard: 1,000, and the Classification code is 6540. THIS IS A SOURCES SOUGHT NOTICE ONLY.
