Description
Statement of Work (SOW) - Request_IHS5416366 **Sources Sought Notice: 5416366** This Sources Sought Notice, issued in accordance with FAR 5.101, is for a contractor to provide services to install 100 pair copper cable for analog lines at the Navajo Area Indian Health Service (NAIHS), Chinle Comprehensive Health Care Facility (CCHCF), Information Resource Management (IRM) Department. This notice is open to all business types. This notice does not commit the government to issue a solicitation or make an award OR to prelude a solicitation expected to be issued from closure of notice. The Chinle Service Unit includes the hospital, CCHCF, and an ambulatory care center in Chinle, AZ, and two satellite health centers; one in Pinon, AZ and the other in Tsaile, AZ. The Chinle Comprehensive Health Care Facility (CCHCF) is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users covering an area 3600 square miles CCHCF is the only medical services in the immediate area. THE ARIZONA DEPARMENT OF HEALTH SERVICES DESIGNATES CCHCF AS A LEVEL IV TRAUMA CENTER, SEPTEMBER 9, 2013 TO SEPTEMBER 9, 2016. In accordance with FAR 5.207 (c) refer to attached Statement of Work (SOW) for a description of the services required. Period of Performance: The Contractor will have 30 days from the time of award to complete the services. Anticipated award date before 09/30/2016.
