Description
This is a combined Synopsis/Solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 13.5, as supplemented with additional information included in this notice. The solicitation number is VA250-16-Q-0156 (Provide Security Enhancements) and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 238210. This requirement is a Service Disabled Veteran Owned Small Business set-aside and only qualified contractors may submit quotes. The VHA NCO 10 - 541 - Louis Stokes VA Medical Center requires: IMPROVEMENTS AND EXPANSION OF THE ELECTRONIC SECURITY MEASURES AT THE FACILITY. The requested items are specified below: 4th Floor Contractor to furnish and install complete, access control to stairwell doors 4B-115, and 4B-149. These two locations currently have manual fire exit push bar style egress hardware. This hardware will need to be replaced/modified to work with electronic controlled hardware that doesn't change the fire door exit function. These doors will not be allowed to be controlled with mag locks. Access control readers are required to be government PIV compliant by HID, 75bit. Contractor to furnish and install complete, access control to the entrance double door set to ward 4B. These doors will need to close at a designated time. To accomplish this the LENEL system will need to interface with the fire alarm door hold openers. These double doors will need to be locked down with maglocks and open after a successful card read. The doors need to be free egress out after hours, so maglocks are to disengage on motion and/or push button from the secure side. Access control readers are required to be government PIV compliant by HID. Contractor to furnish and install complete (6) Axis M3006-V cameras in locations shown on attached floor plan. Two cameras at nurses station, one camera at each entrance to the space. Contractor to furnish and install complete one wall mounted telephone jack at the double door entrance to the 4B ward. Location indicated on the attached floor plan. This phone is to autodial the nurses station for access after hours. Contractor to furnish and install complete, two panic alarm button locations at the nurses station 4B-117. Location to be finalized by the COR. Design locations are shown on the plans. Contractor to furnish and install complete a monitor to view the new camera at the double entry door and existing cameras on the non-secure side of the double door. Contractor to furnish and install a remote release button at the nurses station that will drop the maglocks to the double doors. Contractor to furnish rack mounted LENEL client station (Dell Optiplex). (See attached 4th Floor Drawing for additional information.) 2nd Floor Atrium Contractor to furnish and install complete, access control to the entrance double door set to ACS specialty care vascular lab (see attached drawing). These doors will need to close at a designated time. To accomplish this the LENEL system will need to interface with the fire alarm door hold openers. These double doors will need to be locked down with maglocks and open after a successful card read. The doors need to be free egress out after hours, so maglocks are to disengage on motion and/or push button from the secure side. Access control readers are required to be government PIV compliant by HID, 75bit. Furnish and install LENEL control boards in existing LENEL power cabinet. Basement Atrium Contractor to furnish and install complete, access control to the two double door sets leading into the facility by the prostetics department. (see attached drawing). These doors will need to close at a designated time. To accomplish this the LENEL system will need to interface with the fire alarm door hold openers. These double doors will need to be locked down with maglocks and open after a successful card read on both sides. These doors will need to be equipmed with delayed egress maglocks and door hardware Access control readers are required to be government PIV compliant by HID, 75bit. Furnish and install LENEL power cabinets and control boards for a complete installation. West Skywalk At the current time there is no camera or door securtity for the 1st floor Connecting Cooridor. The contractor shall install security cameras and card swipes to this vital area of the cooridor to reduce the risk for furure security hazards. This scope of work is to have a fully functional security system upon completion. The fire doors are located in this breezeway corridor, directly adjacent to Room 1F550. The door frame label ID is WH-04413. This door needs and automatic door operator 8100 Series or equal with stainless steel push plates, magnetic locks installed as well as a prox card reader for point of entry coming from the parking garage into the facility and a push to exit point of entry leading to the parking garage from the facility. A camera is needed on the exterior side (the point at which someone would be requesting entry), a phone will be installed by the VA Electric Shop to allow contact with police services. The VA has a requirement for NeDap long Range Sensors to be installed for spinal cord patients to have the ability to open these doors. The fire door security magnetic locks must fail safe in the event of a fire alarm. The doors must remain latched to prevent a fire back draft, and allow safe passage of staff and patients. The doors must be equipped with safety motion sensors. The contractor is responsible for all raceways, security equipment, wiring and programming. If any power is required then the contractor is required provide and install as needed. Plenum Composite Cabling to be installed in conduit or cable tray. Please refer to the drawing included with the package. Required on all Floors "The contractor must be Lenel master certified and Dell certified to program and install items to our security network. "All cables are to be marked on both ends at or near the termination point so as to be easily traced in the future. All cables are to be labeled with Brady wire labels or equal. Provide a cable schedule and drawing for all new cables that pertain to the project. All cables and wiring are to be installed in conduit or cable tray. No bridal rings or open wiring will be permitted. "Contractor is to notify the COR when they start and when they are finished at the end of the day. If nursing supervisor or COR requests contractor to discontinue work for the day the contractor must comply. The COR will be notified immediately if there is a work stoppage. The contractor must keep in mind that this is a patient ward and no tools or carts can be left unattended. This will be at no additional time to the government. "Shut downs will be scheduled with the COR. Shut downs will be on Sundays at 6:00A.M. start time. Normal work hours are 7:00A.M. to 3:30P.M.. Off hours can be coordinated with the COR. "The contractor is responsible to fire caulk any penetrations through fire or smoke walls. "Upon completion electric shop/COR is to work in conjunction with the contractor to ensure all devices installed are functioning correctly to the scope of work. "When the scope of work is completed the contractor will give VA Electric Shop (W) employees 4 hours training on the equipment, operation and a briefing on how the equipment was installed. Any special tools, drawings needed to replace equipment or parts will be supplied to the Electric Shop (W) upon completion of the scope of work. ***Request for Information Submissions: Interested contractors must submit questions, via e-mail, concerning the solicitation NLT MARCH 29, 2015 to enable the Contracting Specialist and/or COR to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Quote MUST be good for 120 calendar days. The selected contractor must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. In addition to any VHA terms, the contractor has reviewed and acknowledges any attached solicitation documents, item details, delivery schedule, and/or instructions (collectively referred to as requirements), and agrees to all terms and conditions set forth in the solicitation documents.
