- The VISN 9 Network Contracting Office is conducting market research for the Robley Rex VA Medical Center in Louisville, KY to identify potential sources for a contract within the service area to provide Certified Tumor Registry Services. This sources sought notice is to assist the Louisville VA Medical Center in identifying an appropriate acquisition strategy only. Written communication received will be considered solely for the purpose of determining current market capabilities for the services identified within this notice. This notice does not commit the government to contract for any supply or service. The government will not pay for any information or administrative cost incurred in response to this Sources Sought Notice. Do not contact medical center staff in regards to this requirement, as they are not authorized to discuss matters related to this procurement action. All questions should be submitted in writing for this procurement to the Contract Specialist. 1. REQUIREMENTS a. The purpose of this contract is for the contractor to provide off-site Certified Tumor Registry Services for the Robley Rex VAMC Cancer Registry, 800 Zorn Avenue, Louisville, KY 40206 on an on-demand, per case, as-needed basis, in accordance with the specifications listed herein. b. The period of performance shall be for a base year period plus four (4) one-year options to renew at the discretion of the Government. Contractor is to provide firm-fixed pricing for all years in the Schedule of Items in order to be considered for award. The quantities below are estimates only. Contractor will be paid only for actual services performed. c. Contractor must be familiar with Oncotrax, CPRS, and Vista Imaging for certain reports, i.e. MRIs, etc. d. This procurement is being conducted under Federal Acquisition Regulation (FAR) combination of Part 12, Acquisition of Commercial Items and Part 15, Negotiated Acquisition Procedures. 2. Contractor shall provide Certified Tumor Registry Services for the Robley Rex VAMC Cancer Registry, 800 Zorn Avenue, Louisville, KY 40206 on an on-demand, as-needed basis, as follows: a. Abstracting: Contractor is responsible to transcribe and interpret information from patient records regarding treatment provided to new cancer patients. "Analytical - Cases that were either diagnosed and/or treated at the Robley Rex VAMC (i.e., Class 10, 13,14,20,21,22) "Non-Analytical - Cases that may be picked up at the request of the facility that would not usually qualify for abstracting and would not be considered eligible for sending to the National Cancer Registry and are not subject to follow-up requirements (i.e., severe cervical dysplasia, non-malignant oncocytomas of the kidneys, prostate PIN, and Class of Cases 30-34) "Quality Control - Completed abstracts are reviewed by the Contracting Officer's Representative (COR). Contractor shall correct any mistakes in the registry cases made by them at no cost to the VA. b. Follow-Up: Contractor is responsible for tracking the care all patients received in the years following their cancer diagnosis at the Robley Rex VAMC, Louisville, KY and/or at other health care institutions. c. Casefinding/Suspense: Customer shall generate a suspense list by the VA VISTA OncoTrax Program which indicates possible cancer diagnosis. Contractor shall review this list for possible reporting to the National Cancer Registry and report as necessary including the addition of the appropriate AJCC Stage form. 2. Contractor shall perform all contract services via remote connection to the VA system. Contractor is responsible to provide all equipment and supplies including work areas and computer workstations necessary to complete services required under this contract. Contractor shall be required to ensure set-up with PKI. 3. VA currently uses a Computerized Patient Record System (CPRS) in VISTA. VA will provide contractor with electronic access to CPRS for use by the Certified Tumor Registrar. Contractor employees are responsible to access the system a minimum of once every thirty (30) days. Failure to access VA system at least once every thirty (30) days will result in the contractor's employees being locked out of the VA system. 4. Contractor shall assign a project manager to this contract. VA will assign a COR to this contract. The Contractor's project manager will be available to the VA COR for dealing with the day-to-day issues of the contract. MISCELLANEOUS: 1.The Federal Tort Claims Act does not cover Contractor or Contractor's employees/providers. Contractor shall be responsible to maintain and provide evidence of liability and malpractice insurance covering all Contractors' employees/providers. 2.The Contractor shall be required to have a contingency staffing plan in place to ensure continuity of care as continuity of care is an essential part of the VA's services. The contingency staffing plan shall ensure the Contractor is able to continue performance of services in the event an employee is unable or unwilling to continue performance. 3.The Contractor is responsible to ensure that all on-call services are provided in accordance with contract terms, that all responses are made timely in accordance with contract terms, and that Contractor's employees comply with all credentialing, licensing, and training requirements. 4.Contractor shall be responsible for ensuring that Contractor and all Contractors' employees comply with all Privacy standards and laws, including HIPAA. 5.Contractor shall supervise all Contractors' employees/providers and the VA shall not provide any supervision of Contractor's employees/providers. The VA shall utilize a Quality Assurance Surveillance Plan (QASP) to monitor Contractor's performance and compliance with contract terms. If a qualified and capable small business provider is identified, the Government reserves the right to pursue a sole source contract with the small business provider. Primary NAICS for this procurement will be 541611 (for tumor registry services) with a small business size standard of $15 Million. 4. Documentation relating to capability of performance 5. All interested firms must be registered in SAM (www.sam.gov) - confirm registration or pending registration. 6. If an SDVOSB or VOSB, provide proof of registration in the VA Vet Biz website and verification by VA Center for Veterans Enterprise (CVE) or intention of registration prior to submission of offers to a solicitation; 7. All interested SDVOSB or VOSB firms must be registered in VETBIZ.GOV and Verified by VA CVE and registered in SAM to be eligible for award of Government contracts. The following question is optional.
