Facility Maintenance Scope of Service Oct. 20, 2015 St Augustine VA Outpatient Clinic PERFORMANCE WORK STATEMENT SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary to maintain the building and support equipment defined in this Performance Work Statement (PWS) section for the VA Health Care System St. Augustine Community Based Outpatient Clinic (CBOC) located at 195 South Park Blvd, Saint Augustine, FL 32086.. The contractor shall supply all labor, tools, and materials to perform the below listed tasks. The contractor will also be responsible for coordinating with any subcontractors, VA staff, utility companies, and local governing authority's that may be required to complete repairs: 1. HVAC: a.Every 60 days replace all HVAC filters (22 units). b.Every 120 days clean all HVAC coils and, clean and treat drip pans for mold/mildew. (22 units). c.As needed respond to hot and cold calls (adjust air flow/repair as required) as well as any mechanical HVAC issues. 2.Electrical: a.Replace lightbulbs, fluorescent tubes, and tubes/bulbs in exterior fixtures attached to building as required. b.Replace light fixture ballast as required. c.Trouble shoot electrical failures, repair as required. d.Perform monthly test of the emergency lighting per VA standards. e.Annually replace emergency lighting batteries and test per VA standards 3.Plumbing: a.Respond to all general plumbing issues including but not limited to: clogged drains, busted pipes, water temperature issues, malfunctioning water stops, clogged sink aerators, and malfunctioning flush mechanism on toilets. 4. Fire Protection System: a.Coordinate the quarterly and annual fire alarm and sprinkler inspections with local licensed contractor (Lease owner is responsible for inspection cost). b.Coordinate any repairs with building owner, VA, and Fire Alarm/Sprinkler contractor. c.Ensure that proper documentation is received for all inspections and repairs and is delivered to the VA within 10 working days of completion of inspection/repair. d.Perform monthly visual inspection of portable fire extinguishers and document per VA standards. e.Coordinate the annual fire extinguisher testing with local contractor (Lease owner is responsible for inspection/replacement cost). 5.General/Routine Maintenance: a.Perform monthly inspections of general condition of the building. Generate reports every month and submit to the VA. b.Perform general repairs on building interior and exterior finishes as required. c.Adjust doors and lock assemblies. d.Replace stained ceiling tiles and investigate cause of stain and repair leak. e.Patch/paint wall surfaces as required. f.Replace cracked or loose floor tiles as required. g.Coordinate repairs of systems that require specialty technicians such as fire alarm, intrusion alarm, nurse call, security camera system, auto door openers, and card access control systems. h.Any tasks that require additional parts or labor that are not covered thru this service agreement will be handled separately on a case-by-case basis. 6.TASK FREQUENCY a.The building will be inspected with the COR once every 30 days. This includes the exterior of the building (excluding the parking lot area). b.Change HVAC filters every 60 days. c.Every 120 days clean all HVAC coils and, clean and treat drip pans for mold/mildew. (22 units). d.Respond to emergency calls within 4 hours. e.Emergency repairs completed within 24hrs, 3 working days if parts are required. f.Non-emergency calls respond within 24 hours. g.Non-emergency repairs completed within 5 working days, 10 working days if parts are required. 7. SPECIAL WORK REQUIREMENTS: a.Extreme caution shall be exercised when working. Count tools before entering area and upon exiting to ensure no sharp instruments remain behind after the work is completed. No power tools may be used while children are present in the area. Do not leave any plastic bags within children's reach and be sure gates/doors are closed tightly to prevent children from leaving through open doors/gates. b.All work will be done on a schedule approved by COR. c.All maintenance services shall be performed by trained personnel with required licensing (electrician, Plumbing, HVAC, etc.). d.Upon the completion of work, the contractor shall leave a signed, legible, written report with the COR indicating the date and the work that was completed during the day. If no form is left, it will indicate that no work was accomplished and will result in non-payment. e.The contractor shall ensure a high standard of work is maintained throughout the life of the contract. f.The contractor is to accompany the COR or designated representative once every 30 days on a general inspection of the building. Problems or issues shall be corrected as noted solely by contractor unless otherwise specified. g.All personnel employed by contractor shall wear either a company shirt, or hat bearing the company logo, and a name badge identifying individual and company employed by. The contractor's vehicles shall bear the company name or logo. Proper safety equipment is to be worn at all times. 8.PROPERTY DAMAGE: The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material) will be replaced in a timely manner or corrected by the contractor with like materials and workmanship, at no extra cost to the government, upon approval of the Contracting Officer. 9. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any buildings at the VA and within 50 feet of any building entrance. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Possession of weapons on the premises is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States (Federal) District Court, not a local district state, or municipal court. 10. COMPLIANCE WITH OSHA BLOODBORNE PATHOGENS STANDARD: The contractor shall comply with the Federal/California OSHA Blood borne Pathogens Standard. The contractor shall: A. Have methods by which all employees are educated as to risks associated with blood borne pathogens. B. Have policies and procedures which reduce the risk of employee exposure to blood borne pathogens. C. Have mechanisms for employee counseling and treatment following exposure to blood borne pathogens. D. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, and mouthpieces for the employee during performance of the contract.
