- SF 1449 The South Dakota Army National Guard (SDARNG) has a request to provide job-related Medical Surveillance Examinations and Occupational Health Services to the higher risk full-time support (FTS) and/or temporary personnel who are or could potentially be exposed to health hazards in the work environment. The examination shall include personnel assigned to positions requiring specific standards of physical fitness. There are 2 locations that require this service: • CSMS-1 Mitchell, SD, 57301 • CSMS-2 Rapid City, SD, 57701. All medical surveillance testing desired to be complete by 31 May 2016. No award shall be made unless the Contracting Officer makes confirmation or affirmative determination of responsibility for the potential contractor. Potential contractor must be registered in System for Award Management (SAM) prior to award of contract. Applicable NAICS 321399, Size standard $7.5 million. RFQ solicitation response date is 25 March 2016. Please include the following: 1. Tax ID 2. Completed Clause 252.209-7991(DEV) 3. DUNS # 4. Cage Code 5. Any prompt payment discounts that are offered *Vendors will be paid by Electronic Funds Transfer (EFT) and must invoice Wide Area Work Flow
