- Performance Work Statement Clinical Occupational Health Examination Requirements Guide for Aviation Medical Examiner This is a combined synopsis/solicitation for a commercial service prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number, FA46645320A001 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66, effective 1 April 2013 and the Defense Federal Acquisition Regulations (DFARS) change notice 20130522. It is the contractors' responsibility to be familiar with applicable clauses and provisions. This requirement is being issued as a 100% small business set aside acquisition. The North American Industrial Classification System (NAICS) Code is 621111 with a small business size standard of $11 million. The contractor shall provide non-personal services to include all personnel, equipment, tools, materials, supervision, and other items necessary to perform Civilian Air Traffic Control - Specialist Medical Examinations for applicable personnel stationed at March Air Reserve Base (ARB), California, in accordance with the Performance Work Statement (PWS) which is attached to this notice, and shall become a part of the resulting contract. This requirement is a Performance-Based, Firm Fixed-Price (FFP) Contract. The period of performance will be for a base year and four one (1) year options, to be awarded to the responsible offeror whose proposal conforms to the solicitation and considered to be the most advantageous to the Government pursuant to FAR Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition Procedures, utilizing the Lowest-Price-Technically-Acceptable (LPTA) source selection process. To ensure timely and equitable evaluation of proposals, offerors must comply with instructions contained herein. Proposals must be complete, self-sufficient, and respond directly to the requirements of this RFP. The following factors will be used to evaluate the proposals: (1) Technical Acceptability and (2) Price. Technical Acceptability shall be evaluated as acceptable or not acceptable. Offerors must submit a capability statement addressing how they can meet the requirements of the PWS. A proposal will only be determined technically acceptable if no exception is taken to the PWS and include copies of required documents and certifications as listed in the PWS. Price evaluation shall be based on the lowest reasonable, balanced, and overall total evaluated price. Price will always be an important factor; therefore, offerors should make diligent efforts to control costs and submit fairly priced quotes as each offeror's quoted price will be evaluated for reasonableness. Offers must include a completed Price List that is an attachment to this RFP and will become a part of the resulting contract. The Total Evaluated Price will be the total of all items listed on the price list for the base year and all options. Lack of registration will make an offeror ineligible for award. This notice does not obligate the Government to award the contract; it does not restrict the Government's ultimate approach; nor does it obligate the Government to pay for any quote/proposal preparation costs. Evaluation of proposal may be made without discussions with the offerror(s). I t is also the offerors' responsibility to be familiar with the applicable clauses and provisions that apply to this acquisition. All proposals must be for all items stated, partial proposals will not be considered. Only firm fixed price offers will be evaluated. Award will be made (all or none) to one vendor. L imited Sources Requirement/Geographically Restricting Competition Justification - The contractor shall furnish all personnel, equipment, materials and facilities necessary to perform the civilian air traffic control specialist medical examination services for government civilian federal employees of March ARB, California. All service(s) shall be performed at the contractor's facilities. The contractor's facilities must be located within a radius of 50 miles of March ARB. Air Force Manual (AFMAN) 48-146, Occupational and Environmental Health Program Management mandates all government civilian federal employees (CFE) receive Air Force(AF) required medical examinations and assessments from AF designated health care provider (HCP) at no cost to the CFE. When a Military Treatment Facility (MTF) lacks the resources to perform a required examination, specialty consult, study or laboratory work, the installation's occupational and environmental medicine consultant (IOEMC) may arrange to send the patient to a civilian community (within 25 miles of the base when possible) upon approval of funding from the unit or organization to whom the CFE belongs. The Government's initial market research indicates that the 25 miles radius deem not possible because it significantly limits the pool of qualified small business medical and health facility contractors able to meet the requirement. Therefore, in an effort to increase this pool of qualified contractors and promote competition, the 25 miles is being expanded to the 50 miles radius of March ARB. The treating facility and all medical staff must be within the said 50 miles radius of March ARB. The government reserves the right to conduct a site visit of the facilities being offered on this solicitation before selection of the contractor, and after contract award. Due to the nature of the requirement, the Government reserves the right to do business directly with the contractor. No third party vendor and/or subcontractor will be allowed and considered. Deadline for submission of all proposals shall be no later than 3:00 PM PST on Saturday, 9 April 2016. Submit written proposals; oral proposals will not be accepted. Proposals may be submitted via e-mail or mailed.
