STATEMENT OF WORK (SOW) Cassette Labeler Northport VA Medical Center, Northport NY (632) Nursing Service (118) Telehealth Department 1. Introduction and Overview Our current satellite antenna was damaged in September 2015 beyond repair. We are in need to replace the current antenna with a new one in order to perform Telehealth services from a distant location to either the medical center or other medical centers outside our boundaries. Telehealth is defined as providing clinical care in circumstances where distance separates those receiving services and those providing services. The intent of Telehealth is to provide the Veteran with the right care in the right place at the right time. This is an EMERGENT request because of the nature of the intent. Video-conferencing equipment is one of the most common forms of technologies used in synchronous Telehealth. The satellite is the conduit to facilitate this encounter and promotes the use of synchronous (Real-time) Telehealth which in turn increases access for Veterans to receive care 1.1Background Video-conferencing equipment is one of the most common forms of technologies used in synchronous Telehealth. The satellite is the conduit to facilitate this encounter and promotes the use of synchronous (Real-time) Telehealth which in turn increases access for Veterans to receive care. 1.2 Scope of Work Our scope would be the remove the damaged antenna and replace it with new antenna. Includes labor and travel and does not include disposal of the used antenna. Scope includes a vehicle to lift the new antenna and facilitate removal of the damaged antenna. Scope also includes shipping of the antenna to the facility. 1.3 Objectives 1.The primary objective for procurement of the antenna will be to make Telehealth fully functional on the medical mobile unit 2.The second objective would be to facilitate the right care of the Veteran in the right time and the right place. 1.4Period of Performance- N/A 2. References- N/A 3. Requirements 1.Equipment to be delivered/assembled upon receipt to medical center. 2.Training of staff, minimum one person, to be performed on site. 3.Statement of warranty of equipment, minimum one year. 4.Statement of service included or for purchase: preventive maintenance; service and repair. 3.1 Tasks 1.Deliver one antenna to the Northport VA Medical Center, 79 Middleville Road, Northport, NY. Medical mobile unit 2.Assembly and validation of performance. 3.Training of staff 3.2 End Results/Deliverables- N/A 3.2.1List of Deliverables by Task- N/A 3.3Schedules/Milestones 1.Delivery of one antenna within 30 days of purchase. 2.Validation of performance within 30 days of delivery. 3.4 Other Considerations- N/A 4. Progress/Compliance- N/A 5. Transmittal/Delivery/Accessibility The contractor shall provide any updated information in hard copy and/or electronic form regarding the user's manual for the satellite. 6. Notes- N/A All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM) located on the web at www.sam.gov. It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to this announcement must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at https://www.vip.vetbiz.gov/. All interested Offerors should submit information by e-mail (preferred) to John.Redmond2@va.gov and include the RFQ (RFI) number, title of the project (Automatic Swing Doors Operator), and company name in the email subject heading.
