As prescribed in 17.208(f), insert a clause substantially the same as the following: Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 calendar days. 52.217-9 -- Option to Extend the Term of the Contract. As prescribed in 17.208(g), insert a clause substantially the same as the following: Option to Extend the Term of the Contract (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 15 days. provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years and 6 months. 52.219-28 - Post-Award Small Business Program Rerepresentation. As prescribed in 19.309(c), insert the following clause: Post-Award Small Business Program Rerepresentation (Jul 2013) (a) Definitions. As used in this clause-- Long-term contract means a contract of more than five years in duration, including options. (End of clause) FAR Provision 52.212-2, Evaluation -- Commercial Items (Oct 14), applies to this acquisition and the following evaluation is applicable: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical: Quote must be rated as technically acceptable to be eligible for award. In order to be deemed technically acceptable, the following evaluation criteria must be met: C130 Corrosion Control, Wash, and Lube. The proposal is technically acceptable when it demonstrates the following: (i) Project Director (primary POC) must provide resume with a minimum supervisory work experience of eight (8) years in aircraft corrosion control services including (a) - (e) below. (ii) Site supervisor (alternate POC) must provide resume with a minimum supervisory work experience of five (5) years aircraft corrosion control services to include (a) - (e) below. (iii) Include resumes for all aircraft service personnel providing work under this contract to include experience with (a) - (e) below. Provide resumes for all service personnel to include all related experience with working on aircraft over 85,000 lbs, working knowledge of ground support equipment, proof of citizenship, valid Florida driver's license, and a clean background check. (a) C-130 aircraft experience (1 Year for Project Director/Primary POC) (6 Months for Site Supervisor/Alternate POC) (b) Biohazard aircraft and clean-up experience (c) Obtain secret clearance not later than first day of performance period for all employees under this contract. (If offeror does not currently possess this clearance, offeror must provide the actual methodology/approach for satisfying this requirement) (d) Working knowledge of ground support equipment (e) Working knowledge of maintaining a satellite hazmat program with all MIL spec items Note: Applies to Project Director, Site Supervisors and Service Employees. 2. H-60 Corrosion Control, Wash Experience. The proposal is technically acceptable when it demonstrates the following: (a) H-60 aircraft experience (1 Year for Project Director/Primary POC) (6 Months for Site Supervisory/Alternate POC) (b) Biohazard aircraft and clean-up experience (c) Working knowledge of ground support equipment (d) Working knowledge of maintaining a satellite hazmat program with all MIL spec items Note: Applies to Project Director, Site Supervisors and Service Employees. 3. Experience with Technical Order (TO) 1-1-692. The proposal is technically acceptable when it demonstrates the following: Working knowledge of Air Force Technical Order's 1-1-691,1H-60(H)G-23, 1H-60(H)G-2-1CL-7, 1C-130A-23 and 1C-130A-23CL-1 or equivalent publications in use by other DOD or DOT components operating H-60 or C-130 Aircraft. Note: Applies to Project Director, Site Supervisors and Service Employees (2) Price: Award will be made to the lowest priced technically acceptable offeror. Technical and past performance, when combined, are equal to price. An offeror must be determined to be responsible by the Contracting Officer in order to be eligible for award. Responsibility is described in Federal Acquisition Regulation (FAR) Subpart 9.1, "Responsible Prospective Contractors." Part of the determination addresses financial capability. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options and 6-month extension to the total price for the basic requirement. Government Security Activity and Visitor Group Security Agreements (Mar 12) 5352.242-9000, Contractor Access to Air Force Installations (Nov 12) CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOV 2012) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment.
