This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73. The associated North American Industrial Classification System (NAICS) code for this procurement is 811310, with a small business size standard of $7,500,000.00. The Syracuse VA Medical Center and Rome VA Outpatient Clinic is seeking to purchase service maintenance agreement. All interested companies shall provide quotation(s) for the following: Services A.This firm fixed purchase order (base and 4 option periods) is for service maintenance agreement for four (4) Fulton VTG DF Boilers and one (1) Modsync Controller located at the Syracuse VA Medical Center, 800 Irving Ave, Syracuse, NY 13210 and one (1) Fulton VTG DF Boiler located at the Rome Clinical VA Outpatient Clinic, 125 Brookley Road, Building 510, Rome, NY 13441 from 04/06/2016 to 03/31/2017. All work under purchase order shall be performed by certified technicians from Fulton Boilers Corporation to serve the above mentioned equipment. The service maintenance agreement shall include one (1) annual preventative maintenance and 24/7 onsite emergency response. Vendor shall provide onsite service support 24 hours/7 days a week for the equipment mentioned above. Technicians shall be on site within 24 hours of initial call for boiler and modsync controller malfunctions or failures. Travel, labor and parts costs associated with emergency on-site service calls shall not be considered within the scope of this purchase order, and shall be funded by a separate procurement. Instructions to Offerors For detailed requirements see attachement "Solicitation". All quoters shall submit the following price, technical capability statement, etc (refer to 52.212-2) All offers shall be sent via email to paul.kim8@va.gov. "This is an open-market combined synopsis/solicitation for services as defined herein. The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following: Offeror shall list exception(s) and rationale for the exception(s). Only email submissions shall be accepted no later than March 31, 2016 at 12:00 EST. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). Formal walk-through shall be done at Syracuse VA Medical Center on Monday, March 28, 2016 at 2:00 pm. To attend the walk-through, all vendors shall RSVP via email by March 25, 2016 at 12:00 pm EST.
