Description
The Contractor shall provide 24 hour, 7days a week Basic Life Support (BLS), Advance Life Support (ALS) and Critical Care Transport (CCT) Ambulance service for the VA Connecticut Healthcare System's for beneficiaries of the VA in accordance with the requirements of the contract. The contractor shall provide prices for each type of service in accordance with this contract. This will be sole contract for Ambulance Service for VA Connecticut Health Care System and transport will be to any location VA designates. VA MEDICAL FACILITIES: Upon request, the following facilities will require transportation for eligible veterans for travel from his/her place of origin to authorized locations. Locations to include but are not limited to the following: West Haven VA Medical Center Newington VA Medical Center 950 Campbell Ave 555 Willard Ave West Haven, CT 06516Newington, CT. 06111 All other VA locations associated with the Connecticut VA Healthcare System may be added if needed and considered within scope. Contractor shall provide ALS/BLS/CCT Ambulance service twenty-four (24) hours per day, seven (7) days per week, including all holidays to for beneficiaries of the VA. a.Contractor shall transport patients to or from any pickup point, to or from any floor, ward, nursing home, patient's home, medical center facility, appointment area office or personal belonging that the transporting patients leaving the healthcare facility(e.g. walkers, shower chairs, wheelchairs, suitcase, luggage, boxes) has. b.Contractor shall use contractor-owned stretchers, wheelchairs, liens or any other items required in transporting patient (s) from one place to another. c.This contract shall include Basic Life Support (BLS), Advance Life Support (ALS) and Critical Care Transport (CCT). d.All ambulance vehicles to be used by the Contractor in the performance of this contract must comply with Section 19a-179 (Attachment 1) of the State of Connecticut's Department of Health Services' OEMS Emergency Medical Services Regulations. e.Vehicles shall be made available for inspection when it is requested by the VA. f.The Contractor may be required to furnish more than one ambulance at a time. Vehicle must be able to accommodate all necessary life sustaining equipment during critical care transport and the addition of 1 VA staff member in addition to ambulance personnel. g.In the event of a large number of vehicles requested contractor shall be contacted to determine if he/she can or cannot furnish the vehicles. The COR will record the time of the call, the time service is required, the name of the Contractor's representative and whether or not the Contractor cannot provide the vehicle. If the Contractor cannot provide the added vehicle, the COR will record the name of the ambulance service called which did provide the added vehicle. h.Contractor shall furnish all necessary linen. QUALIFICATIONS: Offers will be considered only from offerors who are regularly established in the business called for, who are financially responsible and have the necessary equipment and personnel to furnish service in the volume required for the items under this contract. The successful bidder (s) shall comply with all requirements, regulations and codes established by this document and those established by Section 19a-179 (attachment 1) of the State of Connecticut's Department of Health Services, OEMS Emergency Medical Services Regulations regarding operations of the services to be provided.
