Description
The Department of Veterans Affairs, VA Medical Center in Orlando is issuing this combined synopsis/solicitation for commodities purchase of various sizes of Stent Implants for interventional radiology procedures for the Lake Nona VA Medical Center located at 13800 Veterans Way, Orlando, FL 32827. It is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The combined synopsis solicitation number is VA248-16-Q-0799. It's issued as a Request for Quotation (RFQ). (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular 2005-85-1 (JAN 4, 2016). (iv) The combined synopsis solicitation is 100% Small Business Set Aside. The NAICS is 339112. Business size standard is 500 employees. (v) This combined synopsis solicitation is for Embolic Embospheres and Quadrasphere stents, Merit Medical Manufacturer, Brand Name product ONLY (Brand Name Justification will be posted IAW FAR 6.305.
