Description
I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All offerors must also be registered on www.sam.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). II. This solicitation is issued as a request for quotation (RFQ); submit written quotes on RFQ number VA246-16-Q-0739. This solicitation is a Brand Name or Equal request for Medisafe America's Sonic Irrigation System; model number PCF Pro 60, to be delivered to the Department of Veterans Affairs (VA) Medical Center at Asheville, North Carolina, which meets the requirements within this solicitation. III. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. IV. This is a total small business set-aside solicitation. The North American Industry Classification System Code is 339113 and the size standard is 750 employees. V. This combined solicitation/synopsis is for purchase of the following commercial supplies which meet the following salient characteristics and requirements of the brand name system. VI. Requirements/Salient Characteristics for Sonic Irrigation System "200-220 V 60Hz electrical Sonic Irrigator system with high pressure sonic irrigation "High temperature thermal rinse cycle "Rotating spray arms in an automated system. Spray arms must be removal. "Must have at least Automated system including: at least 5 automated customizable pre-programmed cycles and automatic enzyme injection "Sonic irrigator must have touchscreen interface to select cycle, start program, end cycle "Automatic fill and drain "Must have transducers placed in top, middle and bottom for maximum cavitation. "Must be capable of: pre-wash, wash, rinse, and air purge for drying lumens. "Must have options for DI or RO water. "Tank to be coated for maximum service life. "Sonic irrigator must have capability to process 3 standard trays of instruments. "Each tray must hold up to 15 lumened instruments "Each tray must hold up to 20 pounds of general surgical instruments. "Must be able to reprocess at least 25 robotic instruments per cycle. "Must be able to clean instruments in vendor trays. "Sonic irrigator must have an automatic lifting and lowering system to load baskets. "Must have option for data port record cycle stats "Sonic Irrigator must easily visually indicate current cycle status. "Must fit into the current footprint of 53" Length X 38 " Width QuantityMPN #Description 1PCF Pro 60Sonic irrigation system which includes: "PCF Pro 60 System "3 standard baskets "3 standard accessory kits "1 - 2.5 gallon bottle of 3E-Zyme "1 Seiko printer "1 roll of printer paper "1 Wash-Checks U Holder and 5 sample cleaning monitors "Installation, validation, and commissioning "Staff in-service VII. Required delivery within 45 days of award. Deliver all products to the VA Medical Center, 1100 Tunnel Road, Asheville, North Carolina 28805. VIII. FAR 52.212-1, Instructions to Offerors -- Commercial Items (OCT 2015), applies to this acquisition. IX. The Government intends to award a firm, fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for proposal, will be most advantageous to the Government. A Lowest Price, Technically Acceptable award will be made based on the evaluation of technical experts. X. The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (MAR 2016), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://sam.gov. XI. FAR clause 52.212-4, Contract Terms and Conditions - Commercial Items (MAY 2015), applies to this acquisition. XII. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (MAR 2016) applies to this acquisition. The following clauses under subparagraph (b) apply: 52.204-10; 52.209-6; 52.219-6; 52.219-28; 52.222-3; 52.222-19; 52.222-21; 52.222-26; 52.222-36; 52.222-50; 52.223-18; 52.225-3 Alternate III; 52.225-13; 52.232-34. XIII. The following VAAR clauses are also applicable to this acquisition: 852.203-70 Commercial Advertising (JAN 2008); 852.211-70 Service Data Manuals (NOV 1984); 852.232-72 Electronic Submission of Payment Requests (NOV 2012); 852.246-70 Guarantee (JAN 2008); 852.246-71 Inspection (JAN 2008). XIV. FAR provision 52.211-6, Brand Name or Equal (AUG 1999) applies to this acquisition. XV. The following VAAR provisions apply to this acquisition: 852.273-70 Late Offers (JAN 2003); 852.273-72 Alternate Evaluation (JAN 2003); and 852.273-74 Award Without Exchanges (JAN 2003). XVI. Quotes are required to be received by the Contracting Officer electronically via email no later than 10:00 A.M. EDT on March 29, 2016. All quotes shall be emailed to darla.hurlock@va.gov. NO FAXED OR MAILED OFFERS WILL BE ACCEPTED.
