- This is a REQUEST FOR INFORMATION notice for market research purposes only to determine the availability of qualified small business sources capable of providing the full range of services described below. Responses should be provided via email to Jen.Johnson@va.gov no later than 3:00pm central time on 04/08/2016. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information to be considered sensitive or proprietary. Respondents will not be notified of the results of the market research. SERVICES: Provide a Community Based Outpatient Clinic (CBOC) in Adams County, Mississippi, for the Department of Veterans Affairs, to include primary care services, mental health support services, laboratory services, radiology services, pharmacy services, tele-health services (personnel and space), etc., using the PACT Staffing Model for a continuum of care from prevention to diagnosis and treatment, to appropriate referral and follow-up. Proposed CBOC contractor shall provide professional medical staff/healthcare providers, medical facilities, equipment and supplies, diagnostic testing and treatment capability, and administrative functions to fulfill the full range of primary care/mental health needs of enrolled Veterans, estimated at 1400 patients. Reimbursement to be made on a per-member/per-month all-inclusive capitated rate. Anticipated contract to be a base 12-month performance period with four (4) one-year options to be exercised at the discretion of the Government. The NAICS code to be utilized for this acquisition will be 621498 - Outpatient Care Centers - and the small business size standard applicable is $20.5M. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1. Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s) and DUNS Number. Proof of Systems for Award Management Registration (SAM) is mandatory. If applicable, please include your GSA schedule, small business status and if applicable, proof as a Veteran Owned Small Business (VOSB) or Service Disabled Veteran Owned Small Business (SDVOSB). VOSB and SDVOSB eligibility must be verifiable in http://www.vetbiz.gov/. 2. A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. Capability Statement shall not exceed more than 3 pages. 3. Submit Capability Statements to: Department of Veterans Affairs, SAO-Central Region Attn: Jennifer M.
