RFQ-0004786_Mod1 PLEASE READ THIS NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Interested parties are responsible for monitoring this site to ensure they have the most up to date information about this acquisition. No partial quotes will be considered. No partial awards will be made. The solicitation number is RFQ0004786 and is issued as a Request for Quote (RFQ). The North American Industrial Classification System (NAICS) code for this requirement is 423830, Industrial Machinery and Equipment Merchant Wholesalers; the size standard is 500 employees. The resultant contract will be a firm-fixed price contract. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-84. This requirement will be awarded on an all or none basis as a commercial item, fixed price purchase order under the procedures at FAR Parts 12 and 13. The FBI requires the following items, Brand Name only, for the following: Line 0001, Qty. 1ea, Vestil Manufacturing Corporation, Part Number WL-100-12-812, Truck Scissor Dock Line 0002, Qty. 1ea, Vestil Manufacturing Corporation, Part Number WL-100-12-S12, Accordion skirt for lift Line 0003, Qty. 1ea, Shipping/Freight Line 0004, Qty. 1ea, Installation, Complete substrate preparation installation Technical Requirements: 1. The lift must have a platform size of 96" x 144". 2. The lift must have a raised height of 72" and lowered height of 14". 3. The lift must have an overall weight limit capacity of 12,000lbs. 4. The lift must have an overall frame size of 86" x 118". 5. The lift must have a push button control valve and a pre-wired control box with a magnetic starter and overload fuse. 6. The lift must have an external 6.5HP 460V, 3-phase, 60 Hz motor. 7. The lift must have a Hydraulic cylinder emergency velocity fuse feature if the line breaks. 8. The lift must have removable 42" handrails and 21" midrails and 4" fixed toe boards. 9. The vendor must provide complete substrate preparation installation of the scissor dock lift according to the manufacturer's instructions. a. The contractor will remove a sufficient amount of exisiting asphalt and base to install footers and a 6" deep concrete pad per manufacturer's specifications. b. The concrete pad will be installed back to the finished asphalt elevation. c. All excavated materials must be removed from the job site. d. The unloading and setting the lift will be provided by the contractor. e. The contractor will be responsible for connecting the three phase power unit. f. The contractor will be responsible for running the hydraulic hoses to the hydraulic power unit (8ft. hoses are supplied with the unit). g. The contractor will furnish the lift platform. h. The contractor will also provide and install one concrete bollard. Shipping: All deliveries under this order shall be FOB destination. Delivery Requirement All items will be shipped to: Federal Bureau of Investigation 4940 Fowler Road Redstone Arsenal, AL 35898 The contractor shall extend to the Government full coverage under a normal commercial warranty. Acceptance of the warranty does not waive the Governments rights with regard to the other terms and conditions of this contract. The warranty period shall begin upon final acceptance of all items provided to the government by the Contractor. Addendum to 52.212-1, Modify (b) Submission of Offers to read as follows: Quotes shall be submitted by email to Kemper Wood, Contract Specialist, kemper.wood@ic.fbi.gov, no later than Thursday, March 31, 2016 at 3:00 PM (Eastern).Quotes shall be clearly marked RFQ0004786. Offerors are hereby notified that if their quotes are not received by the date, time and location specified in this announcement, it will not be considered by the agency. Telephone request will not be considered. No questions will be answered over the telephone. Offers must be valid for 30 calendar days after submission. Please include in your quote: pricing and product description for each line item listed above. Quote may be sent via your company's standard quotation form but must include the following information: Tax ID #, DUNS #, Point of Contact with Email and telephone information. Evaluation Factors: Award will be made to the lowest price/technically acceptable offeror. SAM Requirement Note: Ensure your firm is registered in System for Award Management (SAM) - All vendors must be registered in SAM, effective July 29, 2012, to receive government contracts. The FBI is utilizing a new financial system that has a direct interface with the SAM. If any current FBI vendor data conflicts with SAM Data, the information contained in SAM may override our current information. Please ensure that your company's SAM information is updated and accurate. This includes: TIN, EFT, DUNs, addresses and contact information. The EFT banking information on file in SAM will be what the FBI uses to process payment to your organization.
