- SF 1449 for FA8224-16-T-0018 Single Source Justification Letter This is a solicitation for commercial training supplies prepared in accordance with FAR subpart 13.106-1(b) as supplemented with additional information included in this notice. The attached solicitaion, FA8224-16-T-0018 is issued as a request for quote (RFQ) for skill based change management training curriculum for Hill AFB supervisors, managers, and senior leaders. Submit only written offers: oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) and System for Award Management (SAM). The NAICS code for this effort will be 511130. This solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-87. Please quote for the following item: CLIN Nomenclature 0001 Crucial Conversations 2 Day Participant Suite v4 0002 Crucial Accountability Participant Suite 0003 Influencer Participant Suite v2 0004 Shipping and Handling Delivery Date :On or before 20 May 2016 Contracing Office: AFSC OL:H/PK 6038 Aspen Avenue Hill AFB, UT 84056 Notice to Offerors: The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government closes this solicitation the Government has no obligation to reimburse the offeror for any costs. This requirement is a sole-source acquisition to VitalSmarts (CAGE: 3GL44) for skill-based training curriculum. The Goverment intends to award to this source per the attached single-source justification letter, but will review and consider quotes received from a responsible vendor. All quote(s) must be sent to Peter Nelson via email, peter.nelson.1@us.af.mil or mailed to 6058 Aspen Avenue, BLDG 1289, Hill AFB, UT 84056. Quotes are required to be received no later than 4:30 PM Friday, April 15 2016.
