- Attach 3 - Addenda to 52.212-1 and 52.212-2 Attach 2 - QASP Attach 1 - PWS (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation Number 831505571 is issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (iv) This acquisition is being set-aside for small businesses utilizing the NAICS code 541511 and accompanying small business size standard of $27,500,000. (v) Contract Line Item Number (CLIN) Structure: 0001 - Incident Response Services: Quantity - 1 Unit of Measure - Lot 1001 - Incident Response Services (Option Year 1): Quantity - 1 Unit of Measure - Lot 2001 - Incident Response Services (Option Year 2): Quantity - 1 Unit of Measure - Lot 3001 - Incident Response Services (Option Year 3): Quantity - 1 Unit of Measure - Lot 4001 - Incident Response Services (Option Year 4): Quantity - 1 Unit of Measure - Lot (vi) Description of Requirements: See attached Performance Work Statement (PWS) (Attach 1). (vii) Date(s) and Place(s) of delivery/performance: See attached PWS. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. See addenda to 52.212-1 (Attach 3) to the provision. (ix) The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. See addenda to 52.212-2 (Attach 3) to the provision. (x) Offerors shall ensure that the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items has been met at the System for Award Management (SAM) system. (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Deviation 2013-O0019) (May 2015), applies to this acquisition. (xiii) No additional contract requirements or terms and conditions (contract financing arrangements or warranty requirements) have been determined applicable. (xiv) No Defense Priorities and Allocations System (DPAS) rating is applicable. (xv) Quotations are due on April 13, 2016 at 12:00pm (CDT) and should be submitted to the contract specialist at DITCO via the following email address: timothy.d.ranz.civ@mail.mil. (xvi) Questions or information regarding this combined synopsis/solicitation should be addressed to timothy.d.ranz.civ@mail.mil.
