Description

Seeking Primary Care Physicians - Capt. James A Lovell FHCC The Great Lakes Acquisition Center (GLAC) on behalf of Capt. James A. Lovell Federal Health Care Center, known as the Federal Health Care Center (FHCC) located at 3001 Green Bay Road, North Chicago, IL 60064 is seeking a pool of Board Certified/Board Eligible- Internal medicine/Family Practice Physicians for the Primary Care Directorate. The intent of this Sources Sought Notice is to identify potential vendors that would be interested in providing Internal Medicine or Family Practice Physicians on an "as needed" basis for and at the FHCC and its Evanston CBOC 1942 Dempster Street in Evanston IL 60202, Kenosha CBOC 8207 22nd Avenue, Suite 150 in Kenosha WI 53143, and McHenry CBOC 3715 Municipal Dr in McHenry IL 60050, to cover when vacancies within the Patient Aligned Care Team (PACT). No on-call will be paid. When the "as needed" basis arises, the period of performance could be from 2 weeks to 32 weeks with a guaranteed two (2) week minimum. The normal duty hours will be Monday through Friday from 8:00 a.m. to 4:30 p.m., no weekends, after normal duty hours, or Federal Holidays. The prospective Physicians shall successfully be adjudicated to a mandatory Access National Agency Check and Inquires (ANACI) background investigation prior to performing any services at the FHCC. Once the background investigation has been completed, the FHCC will begin the credentialing and privileging process in order to be VA-approved Physicians. Physicians cannot be scheduled prior to being granted privileges. Current fees associated with the ANACI background investigations are $613.00 per individual. This Sources Sought Notice shall not be considered as a request for proposal or as an obligation on the part of the Government to acquire any products or services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor submission of this Sources Sought Notice or the Government's use of such information. The Government reserves the right to reject, in whole or in part, any contractor's input resulting from this Sources Sought Notice. No contract will be awarded as a result of this announcement. Data submitted in response to this Sources Sought Notice will not be returned. Interested parties shall submit a response in an electronic format only via email to Darryl.scherer@va.gov and Elester.wilbern2@va.gov no later than April 12, 2016 at 10:00 a.m. Please provide the following information: a.Ability to provide the VA approved Physician(s) on "as needed" basis. b.Ability to provide the VA-approved physicians within 30 days. c.Preferred minimum advanced notice when a physician is needed (i.e., 24 hours, 1 week, etc.). d.Provide the range of hourly rates for the physicians on an "as needed" basis. e.Ability to provide physicians for 2 week to 32 week commitments at the FHCC and/or its CBOCS.
