Description
Attachment 3 - Schedule of Items - Additional and updated Items to go with Attachment 1 Amendment 1 Closing Date Extending and Additional Items Added (see Attachment 3). Amendment 1 - Extension and Additional Items added to Schedule of Items. Closing date has been extended to 11:00 AM CDT on Friday, April 1, 2016. See Attachment 3 for additional items added and updated to Schedule of Items (Attachment 1). Please Sign and Return these additional documents with your quote (a copy of this Amendment (SF 30 page 1) and a filled out Attachment 3) prior to the changed closing date. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written request for quotations will not be issued. The Dredge Goetz crew will require grocery deliveries during the dredging season (April - December). This fixed price contract is for furnishing and delivering subsistence and related supplies for the Dredge Goetz vessel to one of the listed delivery sites. Nonperishable groceries/dry goods groceries are typically ordered 2-3 times during the dredging season. Perishable items (fresh produce, dairy, and meats) are typically ordered on a weekly or bi-weekly basis throughout the dredge season. The delivery days and sites are coordinated with the Dredge Goetz and may occur Monday through Friday from 8:00 AM to 5:00 PM. The Dredge Goetz travels the Upper Mississippi River, from Minneapolis, MN to Memphis, TN. Delivery Date of 1st order: April 14-15, 2016. Contractor shall deliver restaurant quality meats and vegetables and retail/consumer sizes of all contracted items. (1) W912ES-16-T-0055 is issued as a request for quotation (RFQ). (2) This RFQ is being conducting using full and open competition - unrestricted under NAICS code 424410 with a business size standard of 250 employees. (3) Contract line item number(s) and items, quantities and units of measure, (including option(s), if applicable): See W912ES-16-T-0055, Attachment 1 and Attachment 2. Please update the spreadsheet labeled Attachment 1 if your item's size, unit choice, and/or unit number differs from what is listed on the spreadsheet. (4) Description of requirements for the items to be acquired: See W912ES-16-T-0055. (5) Date(s) and place(s) of delivery and acceptance and FOB point: See Attachment 2 for the delivery location sites. The dates of orders will be weekly or bi-weekly. FOB Destination. Please put a price for each delivery location listed in Attachment 2. If delivery is included in the item prices, please state so. (6) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (7) The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. Award will be made to the lowest price offer conforming to the RFQ. (8) The provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this acquisition. The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through https://www.acquisition.gov or https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (p) of this provision. (9) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (10) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. There are additional FAR clauses cited in the clause which can be found in W912ES-16-T-0055. (11) Additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices: Update the items listed in Attachment 1 (12 Tabs) to reflect the size, unit type, and unit pack amount that you sell. The award period will be date of award (estimated to be early April 2016) through March 31, 2017, if all options are exercised. The first large order is be delivered on or before April 15, 2016. The attached spreadsheet contains a complete list of items that might be purchased. A purchase of every listed item and the estimated quantities is not guaranteed. Please enter pricing and item information on the attached excel spreadsheet. Quotes submitted in other formats will not be considered unless they are submitted on a similar type of excel spreadsheet. Quotes must be e-mailed to laura.d.nabity@usace.army.mil by the deadline of March 30, 2016 1:00 PM CDT. Late bids will not be accepted.
