- Section L Attachment L5 - Minimum Qualifications Checklist Section L Attachment L4 - Encore III Pricing Template Section L Attachment L2 - Past Performance Information Sheets Section J Attachment G2 - ENCORE III Labor Category Descriptions Attachment L1 - Questions and Answers HC1028-15-R-0030 Amendment 0001 Conformed Copy HC1028-15-R-0030 Amendment 0001 The purpose of Amendment 0001 is to: a. Provide answers to questions in Attachment L1 resulting from the Final RFP dated March 2, 2016, b. Provide revisions to the RFP as a result of the questions and answers, c. Provide a proposal due date extension in block 9 of the SF33 from April 4, 2016 to April 18, 2016, d. Update clauses, and, e. Provide updated attachments G2, L2, L4, and L5. NOTES: The Government is also attaching a conformed copy of the RFP which is the entire solicitation inclusive of the changes from Amendment 0001. The Government will accept additional questions/comments for consideration but a response will not be provided. Please ensure all questions/comments are submitted via the process outlined in the RFP.
