Description

This is a solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested for this solicitation. This solicitation VA250-16-Q-0205 is being issued as a Request for Quotation (RFQ), with the intent to award a Firm-Fixed Price contract for services. All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60. [Firm-Fixed-Price] The contractor shall complete the work required under this solicitation as stated in the Statement of Work (SOW) during the period of performance of June 17, 2016 through June 16, 2017, unless otherwise directed by the Contracting Officer (CO). This requirement is restricted for small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 621512; size standard/$15 million per annum. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, lowest price technically acceptable (LPTA) and received via email to the Point of contact Adoniss Gibson at Adoniss.Gibson@ va.gov. All offers are due by 2:00pm Tuesday, April 05, 2016. The following commercial service is requested: STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Procurement: Sorna eXpedo 24xi Service Contract 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as otherwise specified. The Dayton VA Medical Center may request that service be performed during Non-Standard hours. All necessary labor, management, supervision, technical skills, quality control, materials, parts, tools, transportation, and equipment necessary to perform service and/or preventive maintenance of the Sorna eXpedo 24xi Image Storage System as described below: a.Labor: i.Coverage: Sorna Corporation, via CMX Medical Imaging, agrees to provide maintenance, repair and upgrade services on the Sorna eXpedo 24xi, along with replacement of the entire assembly and accessories per the terms and conditions set forth in the Advanced Exchange Maintenance document. The contractor shall provide unlimited coverage from 8AM - 5PM EST Monday- Friday The contractor shall provide response to vital errors within the shortest possible time. Non-vital failures will be repaired within 24 hours of notification of failure. Diagnostics Software licensing and Service documentation shall be provided. The Dayton VA Medical Center shall receive the preferred labor rate associated with this service agreement. The contractor shall provide remote services, technical telephone support, and clinical telephone support. The contractor shall provide Service management reports which include: Sorna Service Information, Utilization, and quarterly customer loyalty meetings. b.Parts: i. Parts or systems will be new. c.Upgrades/Updates: i.The contractor shall provide software and hardware updates 3.Background: There is currently a service contract that covers, as stated above for the aforementioned systems at this facility. 4.Performance Period: The contract shall be from June 17, 2016 - June 16, 2017. 5.Type of Contract: Firm fixed price. 6.Hours of Coverage: a.Standard business hours are Monday through Friday from 8:00 am to 5:00 pm, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by Chief of Biomedical Engineering, Joy Amundson or her representative. b.Contractor shall not charge extra for working on equipment past 5pm, if the work started before 5pm. c.Federal Holidays observed by the VAMC are: i.New Years' Day ii.Martin Luther King Day iii.Presidents' Day iv.Memorial Day v.Independence Day vi.Labor Day vii.Columbus Day viii.Veterans' Day ix.Thanksgiving Day x.Christmas Day 7.Condition of equipment: The Contractor accepts responsibility for the equipment in "as is" condition. Failure to inspect the equipment prior to contract award will not relieve the Contractor from performance of the requirements of this contract. B. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
