- Security Attachments Solicitation Deviation Technical Personnel Staff List Subcontracting Plan Client Notification Bank Notification Business Management Questionaire Statement Of Work, No Master's Required RFQ-CTS-062-16 CTS Services in Ashland, kentucky Cover Letter The Federal Bureau of Prisons, Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Ashland, Kentucky. The location where services are to be provided should be accessible by local transportation, within 5.0 miles (in the State of Kentucky) of the U.S. Post Office at 2025 29th St., Ashland, KY 41101, and must be located within a 0.5 mile walk from community based transportation. Performance periods under this contract will be for a one-year base period to begin on September 01, 2016 with nine (1) one-year option renewal periods with service ending (if all options are renewed) on May 31, 2026. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, Option Year Five, Option Year Six, Option Year Seven, Option Year Eight, and Option Year Nine respectively.
