Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide and install a Radiation Protection and Suspension System for the William S. Middleton Memorial Veterans Hospital (MVH), 2500 Overlook Terrace, Madison, Wisconsin. The system shall include requirements as described in the salient characteristics below. Suspension System: a.Ceiling track that is no more than 4 meters x 3.5 meters. b.Capability of supporting the spring arms for holding the surgical lighting, shielding, and the injector (between 7kg and 12kg injector). c.Inclusive of a cable spooler that is capable of holding the cabling out of the sterile field and off of the floor. d.Including sufficient cabling to supply power to the light in each lab to allow use along the track. e.Shall be compatible with the Philips Allura F10/10 product. Surgical Lamp: a.Shall be LED technology. b.Shall be surgical quality (100,000 LUX) Acrylic Radiation Shielding on an Adjustable Arm: a.Shielding shall have a lead equivalent of 0.5 mm or better. b.Shielding shall have rounded corners, rounded edging, and be composed of shatter proof materials. The contractor will provide all equipment required. The contractor will install all equipment, associated with the Radiation Protection and Suspension system, installation is required for the week of June 6, 2016. The contractor will remove and dispose of all waste associated with the packaging and set up of this equipment. This requirement will be awarded on a lowest price technically acceptable basis. All interested firms who can meet the requirements stated above should respond, in writing, with: 1.Company name. 2.Address. 3.Point of contact (name/title/telephone number/e-mail address). 4.DUNS number. 5.Tax ID number. 6.Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. 7.A capability statement that addresses the organizations qualifications and ability to provide the brand name or equal vision screener depicted above. Submit responses to the Contract Specialist, Thomas Chesak, at thomas.chesak@va.gov no later than 11:00 a.m. (Central Standard Time), 7 April 2016. Radiation Protection and Suspension System for Madison MVH shall be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 339112, Surgical and Medical Instrument Manufacturing, is applicable to this acquisition; the size standard is 1,000 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
