- Base Access Affidavit Wage Determination Base Access Memo Specifications This is a combined synopsis/solicitation for commercial items prepared under Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented by subpart 13.1 and additional information included in this notice. This announcement constitutes the only solicitation. The North American Industry Classification System (NAICS) code for this agreement is 333996 with a size standard of 1,250 employees. To be eligible for award, offerors at a minimum must provide: -The name, address, telephone number and email address of the offeror with a Point of Contact (POC) to include name, position, telephone number, and email address -Discount terms -Offerors shall submit firm-fixed priced offers according to the below CLINs 0001 Service Call: Field service to check/laser align pump and motor. 0002 The contractor shall furnish all labor, equipment and materials needed to disassemble and inspect, measure and record all critical dimensions, grit blast/vat clean all parts, replace thrust bearings and lip seals, refurbish mechanical seal (lap faces and replace O-rings and gaskets), replace bearing lock nut and washers, polish and reuse shaft and shaft sleeve, balance impeller to G1.0@3600 Pram specifications, assemble complete in accordance with attached specifications. 0003 Service Call: Field service to install back pullout and laser align pump and motor. - In accordance with, FAR 52.212-1(b)(4), offerors are reminded that as a minimum, offers must show a technical description of the items being offered insufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary.. -Prospective offerors must have an active registration in the System for Award Management (SAM) prior to the date and time established for receipt of offers. Offerors who are not registered in SAM prior to the date and time established may be disqualified from the competition. Registration can be completed by going to https://www.sam.gov/portal/public/SAM/#1 and completing the registration process. -In addition to the above requirements, offerors shall complete and submit all required provisions listed below. BADGE REQUEST FOR BASE ACCESS: Contractor shall complete the memorandum and the affidavit attached to this solicitation. The forms will be emailed to gage.bergman@us.af.mil. The memorandum and affidavit will then be submitted to Security Forces by the Contract Specialist. Security Forces will perform a background check on all contractors visiting. When sending emails to the Contract Specialist please check your email options for "obtaining a delivery receipt" to allow verification of the email being delivered. * Ensure the memorandum and the affidavit are emailed/received by the Contract Specialist (at a minimum of) four (4) work days (M-F) before the day of base access so that Security Forces will have enough time to process the required forms and perform the required background check(s). Please be advised do not expect to get on base with a retired military or dependent ID card; Security Forces does not allow the use of these cards when doing business on base. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.
